Description
Overview of the medicine
Clonazepam is a benzodiazepine used to treat various seizure disorders and panic disorder. It has anxiolytic, anticonvulsant, muscle relaxant, and sedative properties.
Uses & Indications
Dosage
Adults
Initial: 0.25 mg orally twice daily for panic disorder; 0.5 mg orally three times daily for seizure disorders. May be increased gradually. Max: 4 mg/day for panic, 20 mg/day for seizures.
Elderly
Lower initial doses (e.g., 0.25 mg daily for panic disorder) and slower titration due to increased sensitivity.
Renal_impairment
No specific dosage adjustment recommended, but use with caution.
How to Take
Take orally with or without food. Swallow whole, do not crush or chew.
Mechanism of Action
Enhances the effect of gamma-aminobutyric acid (GABA) at the GABA-A receptor, leading to increased chloride influx and neuronal hyperpolarization, thus reducing neuronal excitability.

Side Effects
Contraindications
- Acute narrow-angle glaucoma
- Significant liver disease
- Hypersensitivity to benzodiazepines or any component of the formulation
Drug Interactions
Flumazenil
Can reverse the effects of clonazepam.
Carbamazepine
May increase clonazepam clearance.
CNS depressants (opioids, alcohol, other benzodiazepines)
Increased sedative effects, respiratory depression.
Storage

Overdose
Symptoms include drowsiness, confusion, impaired coordination, diminished reflexes, and coma. Management is supportive, including airway management, gastric lavage, and activated charcoal if appropriate. Flumazenil may be considered in severe cases, but with caution due to risk of seizures in benzodiazepine-dependent patients.
