Description
Overview of the medicine
Terazosin is an alpha-1 adrenergic blocker used to treat hypertension (high blood pressure) and benign prostatic hyperplasia (BPH). It works by relaxing blood vessels and the muscles in the prostate and bladder neck.
Uses & Indications
Dosage
Adults
Start with 1 mg once daily at bedtime. May be gradually increased to a maximum of 20 mg/day, but commonly 5-10 mg/day for BPH and 1-5 mg/day for hypertension.
Elderly
No specific dosage adjustment required, but start with a low dose and monitor carefully for orthostatic hypotension.
Renal_impairment
No dosage adjustment necessary for renal impairment.
How to Take
Take orally once daily, usually at bedtime. The first dose and subsequent dose increases should be taken at bedtime to reduce the risk of orthostatic hypotension and syncope.
Mechanism of Action
Terazosin selectively blocks postsynaptic alpha-1 adrenergic receptors, leading to vasodilation (relaxation of blood vessels) and reduced peripheral vascular resistance, thus lowering blood pressure. In BPH, it relaxes smooth muscle in the prostate and bladder neck, improving urine flow.
Pharmacokinetics
Onset
Antihypertensive effect: 15-30 minutes. BPH symptom improvement: generally within 2 weeks, full effect in 4-6 weeks.
Excretion
Approximately 40% excreted in urine and 60% in feces, primarily as metabolites.
Half life
Half-life: 8-14 hours
Absorption
Well absorbed orally (nearly 90%). Peak plasma concentrations are reached in about 1 hour.
Metabolism
Extensively metabolized in the liver via O-demethylation, dealkylation, and conjugation pathways.
Side Effects
Contraindications
- Hypersensitivity to terazosin or other alpha-blockers
Drug Interactions
0
Phosphodiesterase-5 (PDE5) inhibitors (e.g., sildenafil, tadalafil): Increased risk of hypotension.
1
Other antihypertensives (e.g., beta-blockers, diuretics): Additive hypotensive effects.
2
NSAIDs: May reduce antihypertensive effect.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose can cause severe hypotension, characterized by dizziness, lightheadedness, and fainting. Treatment is primarily supportive, including maintaining blood pressure and fluid replacement.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. It is unknown if terazosin is excreted in human milk, use with caution in nursing mothers.

Lab Monitoring
- Blood pressure monitoring
- PSA levels (for BPH patients before and during treatment to rule out prostate cancer)
Doctor Notes
- Counsel patients on the 'first-dose effect' of orthostatic hypotension.
- Advise patients to take the first dose at bedtime and avoid sudden postural changes.
- Monitor blood pressure regularly, especially during dose titration.
Patient Guidelines
- Take the first dose and any increased doses at bedtime to reduce dizziness.
- Avoid sudden changes in position, especially when getting up from a sitting or lying position.
- Inform your doctor about all other medications you are taking.
- Do not stop taking the medicine suddenly without consulting your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
