Description
Overview of the medicine
Terbinafine 1% cream is an antifungal medication used to treat various fungal skin infections such as athlete's foot, jock itch, and ringworm. It works by killing the fungi causing the infection.
Uses & Indications
Dosage
Adults
Apply thinly to the affected area and surrounding skin once or twice daily. Treatment duration is typically 1 week for tinea pedis, tinea cruris, tinea corporis, and cutaneous candidiasis, and 2 weeks for pityriasis versicolor. Continue for a few days after symptoms resolve.
Elderly
No special dosage adjustments needed for elderly patients.
Renal_impairment
No dosage adjustments needed for topical use in patients with renal impairment.
How to Take
Clean and dry the affected skin area thoroughly before applying the cream. Apply a thin layer of cream to the affected skin and the immediate surrounding area, and rub it in gently. Wash hands immediately after application.
Mechanism of Action
Terbinafine inhibits squalene epoxidase, an enzyme in the fungal ergosterol biosynthesis pathway. This leads to an accumulation of squalene within the fungal cell membrane and a deficiency of ergosterol, resulting in fungal cell death.
Pharmacokinetics
Onset
Symptomatic improvement usually begins within a few days; full mycological cure may take 1-4 weeks depending on the infection.
Excretion
Mainly through kidneys and feces for systemically absorbed drug, but systemic excretion is minimal for topical application.
Half life
Local skin concentrations persist for several weeks after cessation of treatment. Systemic half-life after oral administration is 200-400 hours, but not clinically relevant for topical use.
Absorption
Less than 5% systemic absorption after topical application; detectable in the stratum corneum within hours.
Metabolism
Minimal metabolism at the site of application; hepatic metabolism is significant for orally administered terbinafine, but negligible for topical.
Side Effects
Contraindications
- Hypersensitivity to terbinafine or any of the excipients in the cream.
Drug Interactions
Topical
Systemic absorption is minimal, so drug interactions are highly unlikely with topical terbinafine.
Storage
Store below 30°C. Do not freeze. Protect from light and moisture. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause systemic effects due to minimal absorption. If excessive amount is applied, wash it off. In case of accidental ingestion, seek medical advice immediately.
Pregnancy & Lactation
Use during pregnancy and lactation only if clearly needed and under medical supervision, as human data are limited. Consult your doctor.
