Description
Overview of the medicine
Terbinafine is an allylamine antifungal drug used to treat fungal infections of the skin, hair, and nails. It works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes, leading to fungal cell death.
Uses & Indications
Dosage
Adults
Onychomycosis: 250 mg once daily for 6 weeks (fingernails) or 12 weeks (toenails). Tinea infections: 250 mg once daily for 2-4 weeks (tinea pedis/cruris/corporis).
Elderly
No evidence suggesting a need for different dosage in elderly patients with normal renal function.
Renal_impairment
Not recommended for patients with creatinine clearance <50 mL/min or severe renal impairment. Consider dose reduction if used.
How to Take
Administer orally, preferably at the same time each day. Can be taken with or without food.
Mechanism of Action
Terbinafine selectively inhibits squalene epoxidase, an enzyme in the fungal ergosterol biosynthesis pathway. This leads to an accumulation of squalene within the fungal cell and a deficiency of ergosterol, resulting in increased fungal cell membrane permeability and cell death.
Pharmacokinetics
Onset
Clinical improvement may be seen within weeks, but full effect for nail infections can take several months due to slow nail growth.
Excretion
Primarily excreted via urine (about 70-80% as metabolites), with a small amount excreted in feces.
Half life
Terminal half-life is about 200-400 hours (due to redistribution), elimination half-life is around 17 hours.
Absorption
Well absorbed orally (>70%), but extensive first-pass metabolism reduces bioavailability to about 40%.
Metabolism
Extensively metabolized by CYP enzymes (primarily CYP2C9, CYP1A2, CYP3A4, CYP2C8, CYP2D6) in the liver.

Drug Interactions
Caffeine
Terbinafine can reduce the clearance of caffeine.
Cimetidine
May decrease the clearance of terbinafine, increasing its plasma concentration.
Rifampicin
May increase the clearance of terbinafine, decreasing its plasma concentration.
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine)
May increase terbinafine exposure.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include headache, nausea, epigastric pain, and dizziness. Management involves symptomatic and supportive treatment, including gastric lavage and activated charcoal if appropriate.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Terbinafine is excreted in breast milk; therefore, it should not be used by nursing mothers.
