Description
Overview of the medicine
Terbisol (Terbinafine) is an allylamine antifungal used to treat fungal infections of the skin, hair, and nails. It works by preventing the growth of fungus.
Uses & Indications
Dosage
Adults
For onychomycosis, 250 mg once daily for 6 weeks (fingernails) or 12 weeks (toenails). For tinea corporis/cruris, 250 mg once daily for 2-4 weeks. For tinea pedis, 250 mg once daily for 2-6 weeks.
Elderly
No specific dose adjustment needed unless severe renal or hepatic impairment.
Renal_impairment
Reduce dose by 50% if creatinine clearance <50 mL/min.
How to Take
Oral tablets can be taken with or without food. Topical forms should be applied to the affected area once or twice daily.
Mechanism of Action
Terbinafine inhibits squalene epoxidase, an enzyme in the fungal ergosterol biosynthesis pathway. This leads to a deficiency in ergosterol and an intracellular accumulation of squalene, which is toxic to the fungal cell, leading to cell death.
Pharmacokinetics
Onset
Clinical improvement usually seen within weeks, complete cure may take months for nail infections.
Excretion
Primarily excreted in urine (70-80%) as metabolites, small amount in feces.
Half life
Terminal elimination half-life is about 200-400 hours (for tablet).
Absorption
Well absorbed orally, bioavailability around 40% due to first-pass metabolism. Peak plasma concentrations reached within 2 hours.
Metabolism
Extensively metabolized by CYP enzymes (primarily CYP2C9, CYP1A2, CYP3A4, CYP2C8, and CYP2D6) in the liver.
Side Effects
Contraindications
- Hypersensitivity to terbinafine
- Chronic or active liver disease

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
