Overview of the medicine
Terfix (Terbinafine) is an antifungal medication used to treat various fungal infections of the skin, hair, and nails, including dermatophytosis and onychomycosis.
Uses & Indications
Dosage
Adults
Onychomycosis: 250 mg once daily for 6 weeks (fingernails) or 12 weeks (toenails). Tinea corporis/cruris/pedis: 250 mg once daily for 2-4 weeks. Tinea capitis: 250 mg once daily for 4-6 weeks (children's dose varies by weight).
Elderly
No evidence suggests a need for different dosages in the elderly; however, caution is advised if hepatic or renal impairment is present.
Renal_impairment
Not recommended in patients with creatinine clearance < 50 mL/min or severe renal impairment. In moderate renal impairment, consider dose reduction.
How to Take
Terfix tablets should be taken orally once daily with or without food. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Terbinafine inhibits fungal squalene epoxidase, an enzyme involved in fungal sterol biosynthesis. This leads to an accumulation of squalene within the fungal cell and a deficiency of ergosterol, resulting in fungal cell death.

Onset
Clinical improvement typically seen within a few weeks, full effect for nail infections may take months.
Excretion
Primarily excreted in urine (70-75%) and feces (20-25%).
Half life
Terminal half-life is approximately 17-36 hours.
Absorption
Well absorbed orally (>70%), high first-pass metabolism.
Metabolism
Extensively metabolized by at least seven CYP isoenzymes (CYP2C9, CYP1A2, CYP3A4, CYP2C8, CYP2C19, CYP2D6, CYP2B6) to inactive metabolites.

Contraindications
- Hypersensitivity to terbinafine or any component of the formulation
- Chronic or active liver disease
Drug Interactions
Warfarin
Rare reports of changes in INR with concomitant use; monitor INR closely.
Cimetidine
May decrease terbinafine clearance, increasing its plasma levels.
Rifampicin
May increase terbinafine clearance, decreasing its plasma levels.
SSRIs/MAOIs/Beta-blockers
Terbinafine is a CYP2D6 inhibitor and may increase the plasma levels of drugs metabolized by this enzyme.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include headache, nausea, epigastric pain, and dizziness. Management is symptomatic and supportive. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Terbinafine is excreted in breast milk; therefore, it should not be used by nursing mothers.
