Description
Overview of the medicine
Levofloxacin is a synthetic broad-spectrum antibacterial agent of the fluoroquinolone class. It is used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
Usually 500 mg orally once daily for 7-14 days, depending on the type and severity of infection.
Elderly
No specific dose adjustment is required for elderly patients unless they have severe renal impairment.
Renal_impairment
Requires dose adjustment based on creatinine clearance (e.g., lower dose or extended dosing interval for CrCl <50 mL/min).
How to Take
Take orally with or without food. Swallow the tablet whole with a glass of water. Do not crush or chew.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.

Onset
Therapeutic effect observed within 1-2 hours of administration.
Excretion
Primarily excreted unchanged in urine, mainly via glomerular filtration and tubular secretion.
Half life
Approximately 6-8 hours.
Absorption
Rapid and complete oral absorption. Peak plasma concentration reached in 1-2 hours. Bioavailability approx. 99-100%.
Metabolism
Minimally metabolized in the liver, with less than 5% of the dose excreted as metabolites.
Side Effects
Contraindications
- •Hypersensitivity to levofloxacin, other fluoroquinolones, or any component of the formulation.
- •History of tendinitis or tendon rupture associated with fluoroquinolone use.
- •Epilepsy or other seizure disorders.
- •Children and adolescents (due to risk of cartilage damage).
Drug Interactions
Warfarin
May enhance the anticoagulant effects of warfarin. Monitor INR closely.
Corticosteroids
Increased risk of tendinitis and tendon rupture when co-administered with corticosteroids.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Concomitant use may increase the risk of CNS stimulation and convulsive seizures.
Antacids (containing Mg/Al), Sucralfate, Iron/Zinc supplements
May decrease the absorption of levofloxacin. Administer levofloxacin at least 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be considered. Symptomatic and supportive care should be instituted. Levofloxacin is not effectively removed from the body by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Levofloxacin is excreted in breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
