Description
Overview of the medicine
Tetmosol 25% w/v solution is an antiparasitic medication containing Monosulfiram, primarily used for the treatment of scabies and parasitic skin infections. It works by killing the mites that cause scabies.
Uses & Indications
Dosage
Adults
Dilute 1 part Tetmosol solution with 2-3 parts water. Apply to the entire body below the neck, focusing on affected areas, once daily for 5-7 consecutive days. Leave on for 12-24 hours before washing off. Repeat if necessary after a break.
Elderly
Same as adult dosage, with careful monitoring for skin sensitivity.
Renal_impairment
No specific dosage adjustment required for topical use, as systemic absorption is minimal.
How to Take
For external use only. Dilute the solution as directed before application. Apply to clean, dry skin. Avoid contact with eyes, mouth, and other mucous membranes. Do not ingest.
Mechanism of Action
Monosulfiram acts as an ectoparasiticide. When applied topically, it gets absorbed into the skin. After absorption, it inhibits the enzyme aldehyde dehydrogenase in the mites, leading to the accumulation of acetaldehyde, which is toxic to the parasites and ultimately kills them. It also has antifungal and antibacterial properties.
Pharmacokinetics
Onset
Effect usually visible within a few days of consistent application.
Excretion
If systemically absorbed, metabolites are primarily excreted via urine.
Half life
Not well-defined for topical application due to minimal systemic absorption.
Absorption
Limited systemic absorption when applied topically; primarily acts locally on the skin.
Metabolism
Minor systemic metabolism if absorbed, but predominantly local action.
Side Effects
Contraindications
- •Hypersensitivity to Monosulfiram or any component of the formulation
- •Known alcohol intolerance (due to potential disulfiram-like reaction if absorbed systemically and alcohol is consumed)
Drug Interactions
Alcohol
Concurrent use with alcohol (ingested) can lead to a disulfiram-like reaction (flushing, nausea, vomiting, headache, hypotension) due to aldehyde dehydrogenase inhibition, especially if significant systemic absorption occurs. Advise patients to avoid alcohol during treatment.
Other topical preparations
Avoid concurrent use with other topical preparations on the same skin areas to prevent potential interactions or increased irritation.
Storage
Store below 30°C in a cool, dry place, away from direct sunlight. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause severe systemic effects due to minimal absorption. Excessive application may lead to increased skin irritation, redness, and itching. In case of accidental ingestion, seek immediate medical attention. Management is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy and lactation only if clearly needed and the potential benefits outweigh the potential risks. Consult a healthcare professional. Avoid applying to the breast area if breastfeeding.
