Description
Overview of the medicine
Teviral is an antiretroviral medication used for the treatment of HIV-1 infection and chronic hepatitis B virus infection.
Uses & Indications
Dosage
Adults
300 mg orally once daily with food.
Elderly
No specific dose adjustment for elderly patients without renal impairment.
Renal_impairment
Dose adjustment required based on creatinine clearance (CrCl): CrCl 30-49 mL/min: 300 mg every 48 hours; CrCl 10-29 mL/min: 300 mg every 72-96 hours; Hemodialysis patients: 300 mg every 7 days or after a total of ~12 hours of dialysis.
How to Take
Take orally once daily with food. Do not crush or chew the tablet.
Mechanism of Action
Tenofovir Disoproxil Fumarate is converted intracellularly to tenofovir diphosphate, which inhibits reverse transcriptase in HIV and DNA polymerase in HBV, preventing viral replication.
Pharmacokinetics
Onset
Within hours
Excretion
Primarily renal excretion.
Half life
12-18 hours (intracellular half-life of diphosphate is much longer).
Absorption
Rapidly absorbed and converted to tenofovir. Food increases oral bioavailability.
Metabolism
Minimal hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to tenofovir or any component of the formulation.
- Severe renal impairment (CrCl < 10 mL/min) unless on hemodialysis.
Drug Interactions
Didanosine
Increased didanosine levels, requiring dose reduction of didanosine and careful monitoring for adverse effects.
Ledipasvir/Sofosbuvir
Increased tenofovir exposure, monitor renal function.
Nephrotoxic agents (e.g., NSAIDs, aminoglycosides, high-dose/multiple NSAIDs)
Increased risk of renal adverse reactions. Concomitant administration should be avoided.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Management involves symptomatic and supportive treatment, including hemodialysis if necessary (tenofovir is efficiently removed by hemodialysis).
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed and the potential benefit justifies the potential risk to the fetus. Advise HIV-infected mothers not to breastfeed due to potential HIV transmission and risk of adverse effects to the infant. For HBV, consult a specialist.

YesClinical Trials

Lab Monitoring
- Renal function (serum creatinine, CrCl, urinalysis for proteinuria and glycosuria) at baseline and periodically (e.g., every 3-6 months).
- Liver function tests (ALT, AST, total bilirubin) at baseline and periodically.
- Serum phosphate for patients at risk of renal impairment or bone disease.
Doctor Notes
- Monitor renal function (CrCl, serum phosphate, urinalysis) carefully in all patients, especially those with pre-existing renal impairment or risk factors for renal disease. Dose adjustment is critical.
- Screen for hepatitis B (HBsAg, anti-HBc) before initiating Teviral for HIV treatment, and for HIV (HIV antibody) before initiating for HBV treatment. Co-infection management requires careful consideration.
- Advise patients about potential bone effects and the importance of calcium/vitamin D supplementation if indicated.
Patient Guidelines
- Take Teviral consistently with food as prescribed by your doctor.
- Do not stop treatment without consulting your doctor, especially for HBV, as it can lead to severe exacerbations.
- Report any signs of kidney problems (e.g., changes in urination, swelling), bone pain, or unexplained muscle pain/weakness to your doctor immediately.
Missed Dose Advice
If a dose is missed by less than 12 hours, take it as soon as possible with food. If more than 12 hours have passed, skip the missed dose and resume the regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Teviral may cause dizziness; caution advised if driving or operating machinery.
Lifestyle Advice
- Maintain a healthy diet and ensure adequate hydration.
- Avoid excessive alcohol consumption, which can put extra strain on your liver.
