Description
Overview of the medicine
Tezolin 200 mg Tablet is a selective COX-2 inhibitor type of Non-Steroidal Anti-Inflammatory Drug (NSAID) used to relieve pain, inflammation, and stiffness caused by conditions such as osteoarthritis and rheumatoid arthritis. It works by blocking certain enzymes in the body that produce prostaglandins, substances that cause inflammation and pain.
Uses & Indications
Dosage
Adults
For osteoarthritis and acute pain: 200 mg once daily or 100 mg twice daily. For rheumatoid arthritis: 100 mg twice daily or 200 mg once daily.
Elderly
No general dose adjustment needed based on age; however, caution is advised and dose may need to be adjusted based on individual response and renal function.
Renal_impairment
For moderate renal impairment (CrCl 30-60 mL/min), dose reduction may be considered. Not recommended for severe renal impairment (CrCl <30 mL/min).
How to Take
Tezolin tablets should be swallowed whole with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Tezolin selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, it reduces the production of these inflammatory mediators, thereby providing analgesic, anti-inflammatory, and antipyretic effects with a lower risk of gastrointestinal side effects compared to non-selective NSAIDs.
Pharmacokinetics
Onset
Pain relief typically begins within 30-60 minutes.
Excretion
Excreted mainly through urine and feces as metabolites.
Half life
Approximately 8-12 hours.
Absorption
Well absorbed from the gastrointestinal tract, reaching peak plasma concentrations within 2-4 hours after oral administration.
Metabolism
Primarily metabolized in the liver via cytochrome P450 (CYP2C9) enzymes to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to Tezolin or any component of the formulation
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Active gastrointestinal bleeding or ulceration
- •Severe renal or hepatic impairment
- •Severe heart failure
- •Third trimester of pregnancy
Drug Interactions
Lithium
May increase plasma lithium levels.
Diuretics
May reduce the natriuretic effect of diuretics.
Methotrexate
May increase methotrexate toxicity.
ACE inhibitors and ARBs
May reduce the antihypertensive effect and increase the risk of renal impairment.
Warfarin and other anticoagulants
Increased risk of bleeding.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, and epigastric pain. In rare cases, hypertension, acute renal failure, respiratory depression, and coma may occur. Management is primarily supportive and symptomatic. Gastric decontamination (e.g., activated charcoal or gastric lavage) may be considered within four hours of ingestion.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters), Category D (third trimester). Avoid in late pregnancy due to potential for premature closure of the ductus arteriosus and effects on fetal renal function. Consult a doctor before use during lactation; a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
