Description
Overview of the medicine
tget-d-03-01-eye-drop is a combination ophthalmic solution containing an antibiotic (Tobramycin) and a corticosteroid (Dexamethasone) used to treat inflammatory ocular conditions where a bacterial infection or a risk of bacterial infection exists.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the conjunctival sac(s) every 4-6 hours. For severe disease, instill 1-2 drops every 2 hours for the first 24-48 hours, then reduce frequency as inflammation improves. Treatment should not exceed 14 days without reassessment.
Elderly
No specific dosage adjustment required. Use with caution in patients with glaucoma or cataracts, which are more common in elderly.
Renal_impairment
Minimal systemic absorption, so renal impairment is not expected to significantly impact ocular dosing. However, caution advised with prolonged use.
How to Take
For ophthalmic use only. Shake well before use. Avoid touching the dropper tip to any surface to prevent contamination. If using other eye medications, wait at least 5 minutes between applications.
Mechanism of Action
Tobramycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, leading to bacterial cell death. Dexamethasone is a potent corticosteroid that suppresses inflammation by inhibiting phospholipase A2 and stabilizing lysosomal membranes, reducing the release of inflammatory mediators.
Pharmacokinetics
Onset
Anti-inflammatory and antibiotic effects typically observed within hours to a day.
Excretion
Primarily through renal excretion for systemically absorbed tobramycin. Dexamethasone metabolites are excreted renally.
Half life
Systemic half-life for tobramycin is 2-3 hours; for dexamethasone, it is 3-4 hours, but topical ocular half-life is not precisely defined due to minimal systemic absorption.
Absorption
Minimal systemic absorption occurs after topical ophthalmic administration. Tobramycin penetrates ocular tissues well. Dexamethasone also achieves therapeutic concentrations in ocular tissues.
Metabolism
Minimal systemic metabolism for both components after topical ocular administration. Dexamethasone is metabolized in the liver, primarily by CYP3A4, if systemically absorbed.
Side Effects
Contraindications
- Hypersensitivity to any component of the medication
- Viral diseases of the cornea and conjunctiva (e.g., Herpes Simplex Keratitis)
- Mycobacterial infection of the eye
- Fungal infections of ocular structures
- Uncomplicated bacterial infections (steroids can mask infection)
Drug Interactions
Systemic aminoglycosides
Increased risk of systemic toxicity (though rare with topical use).
Other topical corticosteroids
May increase ocular pressure and systemic absorption.
Storage
Store upright at room temperature (15-30°C). Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Topical ocular overdose is unlikely to cause systemic toxicity. If excessive amounts are instilled, the eye may be flushed with lukewarm water. No specific antidote is known.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing mother. Consult a doctor.

Clinical Trials
Extensive clinical trials have established the efficacy and safety of tobramycin and dexamethasone combination for ophthalmic use in various ocular inflammatory and infectious conditions. Studies often compare this combination with individual components or other standard treatments.
Lab Monitoring
- Intraocular pressure (IOP) monitoring with prolonged use (especially >10 days)
- Examination for cataract development with prolonged use
Doctor Notes
- Advise patients to shake the bottle well before use.
- Emphasize the importance of not touching the dropper tip.
- Educate on potential risks of prolonged steroid use in the eye, including IOP elevation and cataract formation.
Patient Guidelines
- Use strictly as directed by your doctor.
- Do not wear contact lenses during treatment, especially if you have an active infection.
- Do not discontinue use suddenly, especially for prolonged treatment, without consulting your doctor.
- Report any worsening of symptoms or new symptoms immediately.
Missed Dose Advice
If a dose is missed, apply it as soon as remembered. If it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Temporary blurred vision may occur after instillation. Do not drive or operate machinery until your vision is clear.
Lifestyle Advice
- Maintain good ocular hygiene.
- Avoid rubbing eyes.
