Description
Overview of the medicine
Cefixime is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It is effective against many Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
Typically 400 mg once daily or 200 mg every 12 hours for 7-14 days, depending on the infection. This 200 mg DS suspension is primarily for pediatric use.
Elderly
Same as adults, but dose adjustment is recommended for severe renal impairment.
Renal_impairment
For creatinine clearance < 60 mL/min, dose adjustment is necessary. For CrCl 21-59 mL/min, 75% of the standard dose; for CrCl < 20 mL/min, 50% of the standard dose.
How to Take
Oral administration. The suspension should be reconstituted with water as directed and shaken well before each use. Can be taken with or without food. Complete the full course of medication even if symptoms improve.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains essential for cell wall strength and rigidity, leading to cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations usually occur 2-6 hours after administration.
Excretion
Eliminated primarily by renal excretion (glomerular filtration and tubular secretion) with some biliary excretion.
Half life
Plasma half-life is typically 3-4 hours, potentially longer in elderly or renally impaired patients.
Absorption
Approximately 40-50% orally absorbed. Food can slightly delay absorption but does not significantly alter the extent of absorption.
Metabolism
Minimal hepatic metabolism; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime, other cephalosporins, or penicillins.
- Patients with a history of severe allergic reactions to beta-lactam antibiotics.
Drug Interactions
Carbamazepine
Elevated carbamazepine levels have been reported when co-administered with cefixime.
Aminoglycosides
Potentially increased risk of nephrotoxicity (though less common with cefixime).
Warfarin and other anticoagulants
May increase prothrombin time (PT) with or without bleeding, requiring INR monitoring.
Storage
Store the dry powder below 30°C, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified period (e.g., 7 or 14 days). Do not freeze.

In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
