Description
Overview of the medicine
Tie 50 mg Tablet is an Angiotensin II Receptor Blocker (ARB) primarily used for the treatment of high blood pressure (hypertension) and heart failure. It helps to relax blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
For hypertension: Initial dose 50 mg once daily. May be increased to 100 mg once daily. For heart failure: Initial dose 50 mg once daily, titrated up to 100-160 mg once daily as tolerated.
Elderly
No initial dosage adjustment is typically required, but individual response should be monitored, and a lower starting dose may be considered.
Renal_impairment
No dosage adjustment is needed for patients with mild to moderate renal impairment. For severe impairment, caution is advised and a lower starting dose may be appropriate.
How to Take
Take Tie 50 mg Tablet orally once daily with or without food. It should be swallowed whole with water.
Mechanism of Action
Tiezoltan selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to decreased blood pressure and reduced cardiac workload.
Pharmacokinetics
Onset
Antihypertensive effect begins within 2 hours.
Excretion
Primarily excreted unchanged in the feces via biliary secretion (about 80%), with a smaller portion excreted in urine (about 20%).
Half life
Elimination half-life is approximately 6-9 hours.
Absorption
Rapidly absorbed after oral administration. Absolute bioavailability is approximately 25%. Food does not significantly affect absorption.
Metabolism
Undergoes minimal hepatic metabolism (less than 10%) by cytochrome P450 enzymes. The primary metabolite is inactive.
Side Effects
Contraindications
- Hypersensitivity to the active substance or any excipients.
- Pregnancy (especially during the second and third trimesters).
- Severe hepatic impairment, biliary cirrhosis, and cholestasis.
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 ml/min/1.73 m²).
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics (e.g., Furosemide)
Increased risk of hypotension and renal dysfunction.
Non-steroidal anti-inflammatory drugs (NSAIDs)
May reduce the antihypertensive effect and increase the risk of renal impairment, especially in elderly, volume-depleted, or renally impaired patients.
Potassium-sparing diuretics (e.g., Spironolactone) and Potassium supplements
Increased risk of hyperkalemia.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include marked hypotension with possible dizziness and syncope. Treatment is symptomatic and supportive. If ingestion is recent, induce emesis or perform gastric lavage. Intravenous fluids may be administered to correct hypotension.
Pregnancy & Lactation
Contraindicated during the second and third trimesters of pregnancy due to risk of fetal injury or death. Not recommended during breastfeeding as it is unknown if it is excreted in human milk; consider discontinuing the drug or breastfeeding.

- Serum potassium levels (especially in patients with renal impairment or those taking potassium-sparing diuretics).
- Renal function tests (serum creatinine, blood urea nitrogen) periodically.
- Liver function tests (baseline and if symptoms suggest hepatic dysfunction).
Doctor Notes
- Counsel patients on the importance of adherence to therapy and lifestyle modifications.
- Regularly assess blood pressure, renal function, and serum potassium levels.
- Avoid co-administration with aliskiren in diabetic or renally impaired patients due to increased risk of adverse events.
