Description
Overview of the medicine
Timolol is a non-selective beta-adrenergic receptor blocking agent used to reduce elevated intraocular pressure in patients with chronic open-angle glaucoma or ocular hypertension.
Uses & Indications
Dosage
Adults
Instill one drop of 0.25% or 0.5% solution in the affected eye(s) twice daily. The dosage should be individualized by a physician.
Elderly
No specific dosage adjustment required, but use with caution due to potential systemic effects and age-related physiological changes.
Renal_impairment
No specific dosage adjustment required for ocular administration.
How to Take
For ophthalmic use only. Instill drops into the conjunctival sac. Avoid contamination of the dropper tip by not touching it to any surface. If using other eye drops, wait at least 5-10 minutes between applications.
Mechanism of Action
Timolol reduces elevated and normal intraocular pressure by decreasing aqueous humor production in the ciliary body, regardless of whether it is associated with glaucoma.
Pharmacokinetics
Onset
20-30 minutes
Excretion
Excreted in the urine as metabolites and unchanged drug.
Half life
Approximately 2.5 to 5 hours systemically. Ocular effects can last up to 24 hours.
Absorption
Systemic absorption occurs after ocular administration. Peak plasma concentrations typically occur within 1 hour.
Metabolism
Primarily metabolized in the liver via cytochrome P450 2D6 (CYP2D6).
Side Effects
Contraindications
- Bronchial asthma or history of bronchial asthma
- Severe chronic obstructive pulmonary disease (COPD)
- Sinus bradycardia
- Second or third degree atrioventricular block
- Overt cardiac failure
- Cardiogenic shock
- Hypersensitivity to timolol maleate or any component of the product
Drug Interactions
Oral beta-blockers
Increased risk of systemic beta-blockade effects, including reduction in heart rate and blood pressure.
Calcium channel blockers
Potentiation of AV conduction disturbances and left ventricular failure.
CYP2D6 inhibitors (e.g., Quinidine, Fluoxetine)
Increased systemic exposure to timolol, potentially enhancing side effects.
Catecholamine-depleting drugs (e.g., Reserpine)
Possible additive effects leading to hypotension and/or marked bradycardia.
Storage
Store at room temperature (below 25°C), away from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Systemic overdose symptoms may include bradycardia, hypotension, bronchospasm, and acute cardiac failure. Treatment is symptomatic and supportive; hemodialysis is not effective.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing woman as timolol is excreted in breast milk.

YesClinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of timolol in reducing intraocular pressure for glaucoma and ocular hypertension. Studies confirm its sustained IOP-lowering effect over 24 hours.
Lab Monitoring
- Intraocular pressure (IOP) regularly
- Heart rate and blood pressure (especially in patients with pre-existing cardiac or respiratory conditions)
- Periodic ophthalmic examinations
Doctor Notes
- Monitor IOP closely during initial therapy and at regular intervals thereafter.
- Exercise caution in patients with a history of pulmonary disease (e.g., asthma, COPD), cardiac conditions (e.g., heart failure, severe bradycardia), or diabetes, due to potential systemic absorption.
- Advise patients on proper instillation technique to minimize systemic absorption and enhance local efficacy. Occlude the nasolacrimal duct immediately after instillation.
Patient Guidelines
- Wash hands thoroughly before and after using the eye drop.
- Do not allow the dropper tip to touch your eye or any other surface to prevent contamination.
- Remove contact lenses before administration and wait at least 15 minutes before reinserting them.
- Do not discontinue the medication without consulting your doctor, even if you feel better.
- Report any unusual or severe side effects to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Timo-Plus eye drop may cause temporary blurred vision or dizziness, especially after initial instillation. Patients should be warned about driving or operating machinery until their vision clears and they are sure they are not experiencing these effects.
Lifestyle Advice
- Maintain regular follow-up appointments with your ophthalmologist to monitor your eye pressure and overall eye health.
- Avoid activities that significantly increase eye pressure, such as heavy lifting or certain inversions, if advised by your doctor.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
