Overview of the medicine
Timolol Maleate ophthalmic solution is a non-selective beta-adrenergic receptor blocking agent indicated for the reduction of elevated intraocular pressure in patients with ocular hypertension or chronic open-angle glaucoma.
Uses & Indications
Dosage
Adults
One drop in the affected eye(s) twice daily.
Elderly
Same as adult dosage, but caution is advised in patients with severe cardiac or pulmonary conditions.
Renal_impairment
No specific dosage adjustment recommended for ophthalmic use.
How to Take
For ocular use only. Tilt head back, pull down the lower eyelid to form a pouch, and instill the prescribed number of drops. Do not touch the dropper tip to the eye or any other surface to avoid contamination. Close eyes gently for 1-2 minutes after administration to minimize systemic absorption.
Mechanism of Action
Timolol Maleate acts by reducing the production of aqueous humor in the ciliary body, thereby lowering intraocular pressure. It does not significantly affect pupil size or visual acuity.

Typically within 20 minutes, maximum effect in 1-2 hours.
Excretion
Primarily via the kidneys as metabolites and unchanged drug.

Side Effects
Contraindications
- •Bronchial asthma or a history of bronchial asthma, severe chronic obstructive pulmonary disease (COPD).
- •Sinus bradycardia, second or third-degree atrioventricular block, overt cardiac failure, cardiogenic shock.
- •Hypersensitivity to Timolol Maleate or any component of the formulation.
Drug Interactions
Oral Beta-blockers
Increased risk of systemic beta-blockade, potentially leading to additive effects on heart rate and blood pressure.
Calcium Channel Blockers
Potential for hypotension and/or bradycardia when co-administered.
Catecholamine-depleting drugs (e.g., Reserpine)
Possible additive effects and the production of hypotension and/or marked bradycardia.
Storage
Store below 30°C. Do not freeze. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of systemic overdose may include bradycardia, hypotension, bronchospasm, and acute cardiac failure. Treatment is symptomatic and supportive; atropine may be used for bradycardia, and beta-agonists for bronchospasm.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution advised during lactation as Timolol is excreted in breast milk and may cause serious adverse reactions in nursing infants. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
