Description
Overview of the medicine
Timomin 0.5% Eye Drop is an ophthalmic solution containing Timolol Maleate, a beta-adrenergic receptor blocking agent. It is primarily used to lower elevated intraocular pressure (IOP) in patients with ocular hypertension or chronic open-angle glaucoma.
Uses & Indications
Dosage
Adults
The usual starting dose is one drop of Timomin 0.5% in the affected eye(s) twice a day. If IOP is adequately controlled, a once-daily regimen may be considered.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised due to potential systemic effects.
Renal_impairment
No specific dosage adjustment is necessary for renal impairment as it is a topical drug, but systemic absorption should be considered in severe cases.
How to Take
For ophthalmic use only. Tilt head back, pull down lower eyelid to form a pouch. Place one drop in the pouch. Do not let the dropper tip touch the eye or any other surface. Close the eye gently for 1-2 minutes.
Mechanism of Action
Timolol Maleate acts as a non-selective beta-adrenergic receptor blocking agent. When applied topically to the eye, it reduces elevated and normal intraocular pressure, primarily by decreasing the production of aqueous humor by the ciliary body.
Pharmacokinetics
Onset
Onset of action usually occurs within 30 minutes, with maximum effect generally occurring within 1 to 2 hours.
Excretion
Metabolites and a small amount of unchanged drug are excreted primarily by the kidneys.
Half life
Plasma half-life is approximately 2-4 hours, but ocular effects can persist longer.
Absorption
Systemic absorption can occur following topical ocular administration. Peak plasma concentrations typically occur 1-2 hours after dosing.
Metabolism
Timolol is extensively metabolized in the liver, primarily by the cytochrome P450 2D6 (CYP2D6) pathway.
Side Effects
Contraindications
- •Bronchial asthma, history of bronchial asthma, or severe chronic obstructive pulmonary disease.
- •Sinus bradycardia, second or third degree atrioventricular block, overt cardiac failure, cardiogenic shock.
- •Hypersensitivity to timolol maleate or any component of the formulation.
Drug Interactions
Oral beta-blockers
Additive effects on intraocular pressure and systemic beta-blockade.
CYP2D6 inhibitors (e.g., quinidine, fluoxetine, paroxetine)
Increased plasma levels of timolol, potentially leading to increased systemic beta-blockade (e.g., decreased heart rate, depression).
Calcium channel blockers, catecholamine-depleting drugs (e.g., reserpine), antiarrhythmics, digitalis glycosides
Additive systemic effects, including hypotension and/or marked bradycardia.
Storage
Store at room temperature (below 25°C) and protect from light. Do not freeze.
Overdose
Symptoms of systemic overdose may include bradycardia, hypotension, bronchospasm, and acute cardiac failure. Treatment should be symptomatic and supportive. An ECG may be performed.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether timolol is excreted in human milk; caution should be exercised when administered to a nursing mother.
