Overview of the medicine
Timonate 5 mg Injection is an antiemetic used for the short-term treatment of nausea and vomiting, especially post-operative or chemotherapy-induced nausea and vomiting.
Uses & Indications
Dosage
Adults
5-10 mg intramuscularly or intravenously, 3-4 times daily as needed. Maximum daily dose 30 mg.
Elderly
Initial dose of 5 mg, then adjust based on response and renal function. Use with caution.
Renal_impairment
Reduce dose by 50% for creatinine clearance <30 mL/min.
How to Take
Administer intramuscularly or intravenously. For intravenous administration, inject slowly over at least 2 minutes. Can be diluted in saline or dextrose solutions.
Mechanism of Action
Timonate acts as a selective dopamine D2 receptor antagonist in the chemoreceptor trigger zone (CTZ) and the gastrointestinal tract, thereby blocking emetic stimuli.

Onset
Within 15-30 minutes (intramuscular), 3-5 minutes (intravenous).
Excretion
Mainly excreted via urine, with a small portion in feces.
Half life
Approximately 4-6 hours.
Absorption
Rapidly absorbed after intramuscular administration, reaching peak plasma concentrations within 30 minutes.
Metabolism
Primarily metabolized in the liver via CYP450 enzymes.
Side Effects
Contraindications
- •Known hypersensitivity to Timonate or any component.
- •Phaeochromocytoma (due to risk of hypertensive crisis).
- •Epilepsy or other conditions predisposing to convulsions.
- •History of tardive dyskinesia from neuroleptics or metoclopramide.
- •Severe hepatic impairment.
Drug Interactions
Anticholinergics
May antagonize the prokinetic effect of Timonate.
Dopaminergic agonists (e.g., bromocriptine)
Timonate may reduce their effect.
CNS depressants (e.g., opioids, benzodiazepines)
May enhance sedative effects.
Drugs that prolong QT interval (e.g., certain antiarrhythmics, antidepressants)
Increased risk of cardiac arrhythmias.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, disorientation, and extrapyramidal reactions. In severe cases, cardiac arrhythmias (including QT prolongation) and seizures may occur. Treatment is symptomatic and supportive. Anticholinergic drugs may be used for extrapyramidal symptoms. Monitor cardiac rhythm.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefit outweighs the potential risk. Excreted in breast milk, so caution should be exercised in nursing mothers and consider alternatives or temporary cessation of breastfeeding.
