Overview of the medicine
Tioxil (Tiotropium Bromide) is a long-acting anticholinergic bronchodilator used to manage chronic obstructive pulmonary disease (COPD).
Uses & Indications
Dosage
Adults
One 18 mcg inhalation capsule once daily using the HandiHaler device.
Elderly
No dosage adjustment required.
Renal_impairment
Use with caution in severe renal impairment (creatinine clearance < 50 mL/min).
How to Take
For oral inhalation only. Do not swallow the capsule. Use only with the HandiHaler device. Puncture the capsule and inhale contents deeply.
Mechanism of Action
Tiotropium is a long-acting, specific muscarinic receptor antagonist, often referred to as an anticholinergic. It acts by inhibiting M3 muscarinic receptors on airway smooth muscle, leading to bronchodilation.

Excretion
Primarily excreted unchanged in urine (about 75% of IV dose). A small portion is excreted via feces.
Half life
Terminal elimination half-life is 5-6 days following multiple once-daily inhalations.
Absorption
Low systemic absorption following inhalation. Oral bioavailability is 2-3%.
Metabolism
Partially metabolized by ester hydrolysis to an alcohol and a carboxylic acid derivative. Cytochrome P450 enzymes play a minor role.
