Description
Overview of the medicine
Tirza 75 mg Injection contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID) used for acute pain and inflammation. It is administered via intramuscular injection and provides rapid relief from moderate to severe pain.
Uses & Indications
Dosage
Adults
Usually 75 mg (1 ampoule) once daily, by deep intramuscular injection into the gluteal muscle. In severe cases, up to 75 mg twice daily may be given, but for no more than 2 days. Total daily dose should not exceed 150 mg.
Elderly
Use with caution; lower doses or longer intervals may be necessary due to increased risk of side effects, especially gastrointestinal bleeding.
Renal_impairment
Use with caution. Avoid in severe renal impairment. In moderate impairment, dosage adjustment may be needed, and monitoring of renal function is advised.
How to Take
For deep intramuscular injection into the gluteal muscle. Intravenous administration should only be done as a slow infusion after dilution.
Mechanism of Action
Diclofenac works by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever.
Pharmacokinetics
Onset
Within 15-30 minutes.
Excretion
Approximately 60% eliminated in the urine as metabolites and less than 1% as unchanged drug; the remainder is eliminated via bile and feces.
Half life
Approximately 1-2 hours (terminal half-life of parent compound), but a longer half-life for its active metabolites (up to 20 hours).
Absorption
Rapidly and completely absorbed after intramuscular administration. Peak plasma concentrations are reached within 10-30 minutes.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation and glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, aspirin, or other NSAIDs
- Active gastrointestinal ulceration, bleeding, or perforation
- Severe heart failure, severe renal failure, or severe hepatic failure
- Third trimester of pregnancy
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Lithium and Methotrexate
Increased plasma levels of lithium and methotrexate.
Diuretics and ACE inhibitors
Reduced antihypertensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management is symptomatic and supportive, including gastric decontamination (if recent ingestion) and monitoring vital signs.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters); Category D (third trimester). Should be avoided in the third trimester due to potential adverse effects on the fetus (e.g., premature closure of ductus arteriosus). Use with caution during lactation; diclofenac is excreted in breast milk.
