Overview of the medicine
Tobramycin and Dexamethasone Ophthalmic Solution is a combination eye drop containing an aminoglycoside antibiotic and a corticosteroid. It is used to treat inflammatory ocular conditions for which a corticosteroid is indicated and where bacterial infection or a risk of bacterial infection exists.
Uses & Indications
Dosage
Adults
Instill one or two drops into the conjunctival sac(s) of the affected eye(s) every 4 to 6 hours. During the initial 24 to 48 hours, the dosage may be increased to one or two drops every 2 hours. Dosage should be gradually decreased as the clinical signs improve.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required as systemic absorption is minimal with topical ophthalmic use.
How to Take
For ophthalmic use only. Shake well before use. Do not touch the dropper tip to any surface to avoid contamination. Tilt head back, pull down the lower eyelid, and instill the drops into the gap created. Close eyes gently for a minute or two.
Mechanism of Action
Tobramycin is a broad-spectrum aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. Dexamethasone is a potent synthetic corticosteroid that suppresses inflammation by inhibiting phospholipase A2 and reducing the release of inflammatory mediators.

Onset
Anti-inflammatory and antibacterial effects can be observed within hours of administration.
Excretion
Tobramycin is primarily excreted unchanged in urine. Dexamethasone metabolites are excreted in urine.
Half life
Ocular half-life not precisely defined for topical administration, but systemic half-life of tobramycin is approximately 2-3 hours and dexamethasone is 3-4 hours.
Absorption
Minimal systemic absorption after topical ophthalmic administration. Tobramycin penetrates ocular tissues. Dexamethasone penetrates ocular tissues; systemic absorption is low but detectable.
Metabolism
Minimal metabolism of tobramycin. Dexamethasone is metabolized primarily in the liver.
Side Effects
Contraindications
- •Hypersensitivity to tobramycin, dexamethasone, or any component of the formulation.
- •Epithelial herpes simplex keratitis (dendritic keratitis).
- •Vaccinia, varicella, and many other viral diseases of the cornea and conjunctiva.
- •Mycobacterial infection of the eye.
- •Fungal diseases of ocular structures.
Drug Interactions
Systemic aminoglycosides
Increased risk of systemic toxicity (e.g., ototoxicity, nephrotoxicity) with significant systemic absorption, although rare with topical ophthalmic use.
Other topical ophthalmic corticosteroids
Increased risk of systemic side effects if significantly absorbed.
Storage
Store at room temperature (below 30°C). Do not freeze. Keep out of reach of children.
Overdose
Overdose is unlikely with topical ophthalmic administration. If an overdose occurs, the eye(s) may be flushed with lukewarm water. Symptomatic treatment is recommended.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Dexamethasone is excreted in human milk; caution should be exercised when administered to a nursing mother. Consult your doctor.
