Description
Overview of the medicine
Tobramycin 0.3% Eye Drop is an antibiotic medication used to treat bacterial infections of the eye, such as conjunctivitis, blepharitis, and keratitis. It belongs to the aminoglycoside class of antibiotics.
Uses & Indications
Dosage
Adults
For mild to moderate infections, instill 1-2 drops into the affected eye(s) every 4-6 hours. For severe infections, instill 2 drops into the affected eye(s) every hour until improvement, then reduce frequency.
Elderly
Same as adult dosage.
Renal_impairment
No dosage adjustment is required for ophthalmic use due to minimal systemic absorption.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch, and instill the prescribed number of drops. Do not touch the dropper tip to the eye or any other surface to avoid contamination. Close the eye gently for 1-2 minutes.
Mechanism of Action
Tobramycin is an aminoglycoside antibiotic that works by binding to the 30S ribosomal subunit of susceptible bacteria, thereby inhibiting bacterial protein synthesis. This leads to cell death.
Pharmacokinetics
Onset
Rapid onset of antibacterial action at the site of infection.
Excretion
Excreted primarily unchanged in the urine if significant systemic absorption occurs.
Half life
Systemic half-life is 2-3 hours; not clinically significant for ophthalmic use.
Absorption
Minimal systemic absorption occurs following topical ophthalmic administration.
Metabolism
Not significantly metabolized after ophthalmic administration.
Side Effects
Contraindications
- Hypersensitivity to tobramycin or any other aminoglycoside antibiotics.
- Known allergic reaction to any component of the formulation.
Drug Interactions
Systemic aminoglycosides
Although systemic absorption is minimal, theoretical risk of increased systemic toxicity if used concurrently with systemic aminoglycosides.
Other topical aminoglycosides
Concurrent use with other topical ophthalmic aminoglycosides may increase the risk of local toxicity.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Ocular overdose can usually be flushed from the eye with lukewarm tap water. Systemic toxicity from ophthalmic overdose is highly unlikely due to minimal absorption.
Pregnancy & Lactation
Pregnancy Category B. While animal studies show no harm, adequate human studies are lacking. Use only if clearly needed. Minimal systemic absorption suggests low risk during lactation, but caution is advised.

Lab Monitoring
- Routine lab monitoring is not typically required for ophthalmic tobramycin due to minimal systemic absorption.
Doctor Notes
- Educate patients on proper ophthalmic instillation technique to maximize efficacy and minimize contamination.
- Advise patients to remove contact lenses before instilling drops and wait at least 15 minutes before reinserting (if allowed).
- Monitor for signs of superinfection, especially with prolonged use.
- Caution patients about potential temporary blurred vision after instillation.
Patient Guidelines
- Do not wear contact lenses during treatment.
- Avoid contaminating the dropper tip with the eye or fingers.
- Complete the full course of treatment even if symptoms improve.
- Inform your doctor if your condition worsens or does not improve within a few days.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply a double dose.
Driving Precautions
This medicine may cause temporary blurred vision after instillation. If this occurs, avoid driving or operating machinery until your vision clears.
Lifestyle Advice
- Maintain good ocular hygiene to prevent reinfection.
- Avoid rubbing your eyes.
