Description
Overview of the medicine
Tolec (Tolperisone) is a centrally acting skeletal muscle relaxant used for the symptomatic treatment of pathologically increased tone of skeletal muscles and muscle spasms. It acts on the central nervous system to inhibit polysynaptic reflex activity.
Uses & Indications
Dosage
Adults
50-150 mg orally, three times daily. The dose should be adjusted according to individual needs and tolerability.
Elderly
No specific dose adjustment is generally required, but caution is advised. Individual dose adjustment may be necessary based on renal/hepatic function.
Renal_impairment
In patients with moderate renal impairment, dose should be reduced and/or interval increased. Not recommended in severe renal impairment.
How to Take
Tolec tablets should be taken orally, preferably after meals, with a glass of water.
Mechanism of Action
Tolperisone acts by stabilizing nerve membranes, thereby reducing the excitability of primary afferent neurons and motor neurons. It inhibits voltage-gated sodium channels and voltage-gated calcium channels, which leads to a reduction in neurotransmitter release and inhibition of polysynaptic reflexes.

Onset
Onset of action typically occurs within 1-2 hours.
Excretion
Excreted primarily through the kidneys, almost entirely as metabolites.
Half life
Elimination half-life ranges from 1.5 to 2.5 hours.
Absorption
Well absorbed from the gastrointestinal tract, but undergoes extensive first-pass metabolism. Peak plasma concentrations are reached approximately 0.5-1 hour after administration.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP2D6) enzymes, to inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to tolperisone or to any of the excipients.
- Myasthenia gravis.
- Children below 18 years (due to lack of sufficient data).
Drug Interactions
Niflumic acid
Concomitant use with niflumic acid may increase the concentration of niflumic acid, especially in children, requiring caution.
Psychotropic drugs
Caution is advised when co-administering with psychotropic drugs, as potentiation of effects may occur.
Other muscle relaxants
Concurrent use with other centrally acting muscle relaxants may potentiate their effects, requiring dose adjustment.
Storage
Store below 30°C, in a dry place, protected from light. Keep out of reach of children.

Symptoms of overdose may include drowsiness, dizziness, gastrointestinal symptoms (nausea, vomiting, abdominal pain), and in severe cases, seizures or ataxia. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered within a short period after ingestion.

Pregnancy: Not recommended during pregnancy, especially in the first trimester, unless clearly necessary and potential benefits outweigh risks. Lactation: Not recommended during lactation as it is not known whether tolperisone is excreted in human milk.
