Overview of the medicine
Tolfenamic acid is a non-steroidal anti-inflammatory drug (NSAID) used to treat pain, inflammation, and fever. It is particularly effective for migraine and tension headaches, dysmenorrhea, and musculoskeletal disorders.
Uses & Indications
Dosage
Adults
For acute migraine, 200 mg at onset, repeat after 1-2 hours if needed (max 400 mg/day). For other conditions, 200 mg 1-2 times daily.
Elderly
Use with caution; lower doses may be required.
Renal_impairment
Reduced dose or avoided in severe impairment.
How to Take
Take orally, preferably with or after food to minimize gastrointestinal discomfort.
Mechanism of Action
Tolfenamic acid inhibits prostaglandin synthesis by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing pain, inflammation, and fever.

Primarily excreted via urine (about 60%) and feces (about 40%) as metabolites.

Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
Diuretics and ACE inhibitors
Reduced antihypertensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Other NSAIDs or corticosteroids
Increased risk of gastrointestinal side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, epigastric pain, lethargy, drowsiness, and gastrointestinal bleeding. Management involves symptomatic and supportive treatment, gastric lavage, and activated charcoal if overdose is recent.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to potential fetal harm (premature closure of ductus arteriosus). Use with caution in the first and second trimesters only if clearly needed and the potential benefit outweighs the risk. Tolfenamic acid is excreted in breast milk; avoid use during lactation or discontinue breastfeeding.
