Description
Overview of the medicine
Topibet-CL 0.05%-1% Cream is a combination medicine containing a potent corticosteroid (Clobetasol Propionate) and an antifungal agent (Clotrimazole). It is used to treat various inflammatory and itchy skin conditions that are complicated by fungal infections.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area twice daily, morning and evening, for up to 7 days or as directed by the physician. Do not use for more than 2 weeks without medical review.
Elderly
Same as adult dosage, use with caution due to thinner skin.
Renal_impairment
No specific dose adjustment required for topical application, but caution in severe cases if significant systemic absorption is expected.
How to Take
For external use only. Wash and dry the affected area before applying a thin film of cream. Gently rub until absorbed. Wash hands immediately after use unless hands are the treated area. Avoid contact with eyes, nose, mouth, and other mucous membranes.
Mechanism of Action
Clobetasol Propionate acts as a potent topical corticosteroid, reducing inflammation, itching, and redness. Clotrimazole is an azole antifungal that works by inhibiting the biosynthesis of ergosterol, an essential component of the fungal cell membrane, leading to increased cell permeability and leakage of cellular contents, thereby inhibiting fungal growth.
Pharmacokinetics
Onset
Relief of symptoms typically within a few days of application.
Excretion
Excreted mainly via urine and feces for systemically absorbed components.
Half life
Not well-defined for topical application; systemically absorbed clobetasol has a half-life of about 3-5 hours.
Absorption
Minimal systemic absorption through intact skin; absorption increases with inflammation, occlusion, or application to large areas.
Metabolism
Primarily hepatic metabolism for systemically absorbed components.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate, Clotrimazole, or any component of the cream
- Viral skin infections (e.g., herpes simplex, varicella)
- Tuberculosis of the skin
- Acne vulgaris
- Rosacea
- Perioral dermatitis
- Diaper rash
Drug Interactions
N/A
Systemic drug interactions are unlikely due to minimal topical absorption.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Acute overdose is unlikely with topical application. Prolonged excessive use may lead to systemic effects of corticosteroids, such as Cushing's syndrome, hyperglycemia, and glucosuria. In such cases, discontinue the cream gradually under medical supervision and manage symptoms supportively. Systemic absorption of clotrimazole is negligible.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus (Clobetasol is Category C). For lactation, it is not known whether topical administration of corticosteroids could result in sufficient systemic absorption to produce detectable quantities in breast milk. Use with caution and only if clearly needed.

Clinical Trials
No specific clinical trial information is typically available for generic combination topical creams; efficacy and safety are extrapolated from individual active ingredients.
Lab Monitoring
- Not typically required for short-term, localized topical use. For extensive or prolonged use, monitoring for systemic corticosteroid effects (e.g., blood glucose, adrenal function) may be considered.
Doctor Notes
- Emphasize short-term use (max 7 days, review for up to 2 weeks).
- Counsel patients on proper application technique and avoiding face/groin/axilla.
- Be aware of potential for systemic effects with extensive use, especially in children.
Patient Guidelines
- Use only as directed by your doctor.
- Do not use for longer than prescribed, typically not more than 7 days.
- Avoid applying to the face, groin, or armpits unless specifically instructed by your doctor.
- Do not cover the treated area with bandages or other dressings unless directed by your doctor.
- Wash hands before and after applying the cream.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
No known effects on ability to drive or operate machinery.
Lifestyle Advice
- Maintain good skin hygiene.
- Wear loose-fitting clothing to avoid irritation.
- Avoid scratching the affected area to prevent worsening of the condition or secondary infection.
