Description
Overview of the medicine
Topicort 0.1% Cream (desoximetasone) is a high-potency topical corticosteroid used to relieve the inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses.
Uses & Indications
Dosage
Adults
Apply a thin film of Topicort 0.1% Cream to the affected skin area two or three times daily as directed by a physician. Rub in gently.
Elderly
No specific dosage adjustment recommended; use with caution due to potentially thinner skin and increased susceptibility to side effects.
Renal_impairment
Not applicable for topical use due to minimal systemic absorption.
How to Take
For topical dermatologic use only. Not for ophthalmic, oral, or intravaginal use. Apply a thin film to the affected skin areas and rub in gently. Do not apply to the face, groin, or axillae unless directed by a physician. Avoid occlusive dressings unless specifically instructed by a doctor.
Mechanism of Action
Desoximetasone works by inducing phospholipase A2 inhibitory proteins, called lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Onset
Clinical effects typically observed within hours to days of initiation.
Excretion
Excreted primarily by the kidneys, with some biliary excretion, if systemically absorbed.
Half life
Not definitively established for topical use due to minimal systemic absorption; however, absorbed corticosteroids are metabolized and excreted.
Absorption
Systemic absorption varies. Factors influencing absorption include the vehicle, the integrity of the epidermal barrier, and the use of occlusive dressings. Absorption is increased by occlusion and in areas with thin skin or where the skin barrier is compromised.
Metabolism
Primarily metabolized in the liver, if systemically absorbed.
Side Effects
Contraindications
- Hypersensitivity to desoximetasone or any component of the formulation.
- Untreated viral infections (e.g., herpes simplex, varicella).
- Untreated fungal or bacterial skin infections.
- Rosacea and perioral dermatitis.
Drug Interactions
Systemic absorption
Due to minimal systemic absorption, clinically significant drug interactions are unlikely with topical desoximetasone.
Storage
Store at controlled room temperature 20°C to 25°C (68°F to 77°F). Excursions permitted to 15°C to 30°C (59°F to 86°F). Protect from freezing. Keep out of reach of children.
Overdose
Topically applied corticosteroids can be absorbed in sufficient amounts to produce systemic effects (e.g., hypercorticism, adrenal suppression). If systemic toxicity occurs, discontinue the drug gradually and treat symptomatically.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether topical administration of corticosteroids could result in sufficient systemic absorption to produce detectable amounts in breast milk. Use with caution in nursing mothers; advise against applying to the breast area.

Clinical Trials
Desoximetasone has been extensively studied in clinical trials for its efficacy and safety in various inflammatory dermatoses, demonstrating significant improvement in symptoms compared to placebo or other topical steroids of lower potency.
Lab Monitoring
- If significant systemic absorption is suspected (e.g., with extensive or prolonged use, especially in children or with occlusion), monitor for hypothalamic-pituitary-adrenal (HPA) axis suppression (e.g., plasma cortisol levels).
Doctor Notes
- Educate patients on proper application technique and emphasize the importance of limiting treatment duration, especially on sensitive areas or in children.
- Advise patients about potential local side effects and to report any signs of infection or severe skin changes.
- Consider tapering off if used for prolonged periods to avoid rebound phenomena.
Patient Guidelines
- Use exactly as prescribed by your doctor; do not use for longer than the prescribed duration.
- Wash hands before and after applying the cream.
- Apply a thin layer to the affected areas only.
- Avoid contact with eyes, mouth, and other mucous membranes.
- Do not bandage, cover, or wrap the treated skin area unless directed by the doctor, as this may increase systemic absorption.
Missed Dose Advice
Apply the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double doses.
Driving Precautions
Topical desoximetasone is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Keep the skin hydrated with non-medicated moisturizers, especially after bathing.
- Avoid known irritants or allergens that can trigger skin conditions.
