Description
Overview of the medicine
Topidin (Esomeprazole) is a proton pump inhibitor used to treat conditions involving excessive stomach acid production, such as GERD, peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily for 7-10 days (with antibiotics).
Elderly
No specific dose adjustment is required, but caution should be exercised due to potential age-related organ function decline.
Renal_impairment
No dose adjustment is generally necessary for patients with renal impairment. Use with caution in severe renal impairment.
How to Take
Take at least one hour before a meal. Swallow capsules whole; do not chew or crush. For patients with difficulty swallowing, the capsule can be opened and granules mixed with water or fruit juice (do not crush granules).
Mechanism of Action
Esomeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby reducing gastric acid secretion.

Within 1 hour of administration.
Excretion
Excreted primarily in urine (80%) and feces (20%) as metabolites.
Half life
Approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability is about 64% after a single dose and increases to 89% with repeated administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Nelfinavir
Significant decrease in nelfinavir plasma concentrations, leading to reduced efficacy. Concomitant use is contraindicated.
Tacrolimus
Increased tacrolimus plasma concentrations.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased serum levels of methotrexate, particularly high-dose methotrexate.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include gastrointestinal disturbances, weakness, and central nervous system effects. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is unknown if esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
