Adults
Epilepsy: Initial dose typically 25 mg once daily for one week, then titrate by 25-50 mg/day weekly to a target dose of 100-400 mg/day. Migraine Prophylaxis: Initial dose 25 mg once daily for one week, then increase by 25 mg/day weekly to a target dose of 100 mg/day.
Elderly
Initiate at lower doses and titrate slowly, monitoring for adverse effects, especially renal function.
