Overview of the medicine
Topril 50 mg Tablet is an angiotensin-converting enzyme (ACE) inhibitor used to treat high blood pressure (hypertension), heart failure, and to reduce the risk of cardiovascular events in patients at high risk. It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Hypertension: Initial dose 50 mg once daily. May be increased up to 100 mg once daily if needed. Heart failure: Initial dose 25 mg once daily, titrated up to 50-100 mg once daily as tolerated. Maximum daily dose 200 mg.
Elderly
Start with a lower dose (e.g., 25 mg once daily) and titrate carefully based on response and renal function.
Renal_impairment
Dosage adjustment is necessary. For creatinine clearance <30 mL/min, consider reducing the initial dose to 25 mg once daily and titrate cautiously. Close monitoring of renal function and potassium levels is essential.
How to Take
Topril 50 mg Tablet should be taken orally once daily, with or without food. It is recommended to take it at approximately the same time each day.
Mechanism of Action
Topril inhibits the angiotensin-converting enzyme (ACE), which is responsible for converting angiotensin I to angiotensin II. Angiotensin II is a potent vasoconstrictor and stimulates aldosterone secretion. By inhibiting ACE, Topril reduces angiotensin II levels, leading to vasodilation, decreased aldosterone secretion, reduced sodium and water reabsorption, and consequently, a reduction in blood pressure and cardiac workload.

Contraindications

Drug Interactions
NSAIDs

Lithium

Diuretics
Increased hypotensive effect. Monitor blood pressure closely.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia. Avoid concomitant use or monitor potassium levels frequently.
Dual blockade of the RAAS (e.g., with ARBs or Aliskiren)
Increased risk of hypotension, hyperkalemia, and renal impairment. Not recommended.

Symptoms of overdose include severe hypotension, shock, bradycardia, electrolyte disturbances, and renal failure. Treatment involves symptomatic and supportive measures, including intravenous fluid administration for hypotension and hemodialysis if necessary.
Pregnancy & Lactation
Topril is contraindicated during the second and third trimesters of pregnancy due to risks of fetal injury and death. Discontinue immediately if pregnancy is detected. It is not recommended during breastfeeding as it may pass into breast milk and harm the infant.
