Description
Overview of the medicine
Torax 10 mg Tablet contains Torasemide, a loop diuretic used to treat edema associated with congestive heart failure, renal disease, and hepatic cirrhosis. It is also used for the treatment of hypertension.
Uses & Indications
Dosage
Adults
For edema: Usually 5-20 mg once daily. For hypertension: Usually 2.5-5 mg once daily. The dose can be adjusted based on clinical response.
Elderly
Similar to adult dosage, but titration should be done carefully due to potential for decreased renal function.
Renal_impairment
No specific dose adjustment is required for mild to moderate renal impairment, but caution is advised. For severe renal impairment, lower doses or extended intervals may be necessary.
How to Take
Oral administration. Can be taken with or without food, usually in the morning to avoid nocturnal diuresis.
Mechanism of Action
Torasemide acts on the thick ascending limb of the loop of Henle in the kidney to inhibit the Na+/K+/2Cl- cotransporter, leading to increased excretion of sodium, chloride, and water.

Onset
Diuretic effect begins within 1 hour after oral administration.
Excretion
Mainly excreted by the kidneys (approximately 80% as metabolites and 20% as unchanged drug).
Half life
Approximately 3.5 hours in healthy individuals; may be prolonged in renal or hepatic impairment.
Absorption
Rapidly and almost completely absorbed after oral administration. Bioavailability is approximately 80-90%. Peak plasma concentrations are achieved within 1 hour.
Metabolism
Metabolized in the liver by cytochrome P450 enzymes, primarily CYP2C9, to inactive metabolites.

Contraindications
- Hypersensitivity to torasemide or sulfonylureas
- Anuria
- Hepatic coma or pre-coma
- Hypotension
- Severe electrolyte depletion
Drug Interactions
Digoxin
Increased risk of digoxin toxicity due to hypokalemia.
Lithium
Reduced renal clearance of lithium, leading to increased lithium levels and toxicity.
Corticosteroids
Increased risk of hypokalemia.
Aminoglycoside antibiotics
Increased risk of ototoxicity and nephrotoxicity.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce diuretic and antihypertensive effects of torasemide.
