Description
Overview of the medicine
Torosic 10 mg Tablet contains Torsemide, a loop diuretic used to treat fluid retention (edema) associated with heart failure, kidney disease, or liver disease, and high blood pressure.
Uses & Indications
Dosage
Adults
Edema: Initially 5-20 mg once daily, adjusted based on response up to 200 mg daily. Hypertension: Initially 2.5-5 mg once daily, adjusted based on response up to 10 mg daily.
Elderly
No specific dose adjustment generally required, but close monitoring is advised due to increased risk of electrolyte imbalance.
Renal_impairment
Dose adjustment may be necessary in severe renal failure. Max 200 mg daily.
How to Take
Take orally, with or without food, usually once daily in the morning.
Mechanism of Action
Torsemide acts on the thick ascending limb of the loop of Henle in the kidneys, inhibiting the sodium-potassium-chloride cotransporter (NKCC2). This prevents the reabsorption of sodium, chloride, and water, leading to increased urine output.
Pharmacokinetics
Onset
Within 1 hour (oral).
Excretion
Primarily excreted via the kidneys (approx. 80% as metabolites, 20% unchanged drug).
Half life
Approximately 3.5 hours. Can be prolonged in hepatic or renal impairment.
Absorption
Rapidly and almost completely absorbed from the GI tract. Bioavailability is ~80-90%. Peak plasma concentrations achieved within 1 hour.
Metabolism
Primarily metabolized in the liver by cytochrome P450 2C9 (CYP2C9) to inactive and weakly active metabolites.
Side Effects
Contraindications
- Anuria (absence of urine production)
- Hypersensitivity to torsemide or sulfonylureas
- Hepatic coma and pre-coma states
Drug Interactions
NSAIDs
May reduce diuretic and antihypertensive effects of torsemide.
Digoxin
May increase the risk of digoxin toxicity due to electrolyte imbalance (hypokalemia).
Lithium
May reduce renal clearance of lithium, leading to increased lithium levels and toxicity.
Aminoglycosides/Ethacrynic acid
Increased risk of ototoxicity.

Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms may include dehydration, electrolyte depletion (hypokalemia, hyponatremia), hypotension. Management involves symptomatic and supportive care, fluid and electrolyte replacement.
