Description
Overview of the medicine
Torsid 5 mg tablet contains Torsemide, a loop diuretic, used to treat edema associated with congestive heart failure, renal disease, or hepatic disease. It is also used to treat hypertension.
Uses & Indications
Dosage
Adults
For edema: Initially 5 mg or 10 mg once daily. For hypertension: Initially 2.5 mg or 5 mg once daily. Dosage may be adjusted based on patient response, up to a maximum of 200 mg daily.
Elderly
Similar to adults, but careful monitoring of fluid and electrolyte balance is recommended due to increased susceptibility.
Renal_impairment
Dosage adjustment may be necessary. Use with caution in severe renal impairment, particularly in anuria.
How to Take
Take orally, with or without food, preferably in the morning to avoid nocturnal diuresis. Swallow the tablet whole with water.
Mechanism of Action
Torsemide acts on the ascending limb of the loop of Henle in the kidney, inhibiting the Na+/K+/2Cl− cotransport system, which reduces the reabsorption of sodium, potassium, and chloride ions. This leads to increased excretion of water, sodium, and chloride, resulting in a diuretic effect.

Within 1 hour (oral).
Excretion
Mainly excreted in the urine, with approximately 80% as metabolites and 20% as unchanged drug.
Half life
Approximately 3.5 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; bioavailability is approximately 80%.
Metabolism
Primarily metabolized in the liver by the cytochrome P450 system (CYP2C9).
Side Effects
Contraindications
- •Hypersensitivity to torsemide or sulfonylureas
- •Anuria (absence of urine production)
- •Hepatic coma and pre-coma states
- •Severe electrolyte depletion
Drug Interactions
Lithium
Reduced renal clearance of lithium, leading to increased serum lithium levels and toxicity.
ACE inhibitors and ARBs
Increased risk of hypotension and renal dysfunction, especially with initial doses.
Aminoglycoside antibiotics
Increased risk of ototoxicity.
Cardiac glycosides (e.g., Digoxin)
Hypokalemia induced by torsemide may enhance digitalis toxicity.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the diuretic and antihypertensive effects of torsemide.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include profound diuresis, resulting in dehydration, electrolyte depletion (hypokalemia, hyponatremia), and hypotension. Management involves symptomatic treatment and correction of fluid and electrolyte imbalances.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk, but human studies are lacking. Use during pregnancy only if clearly needed. It is not known whether torsemide is excreted in human milk; use with caution in nursing mothers.
