Description
Overview of the medicine
Tory 60 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and reduce inflammation in conditions like osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and gout. It works by selectively inhibiting the COX-2 enzyme.
Uses & Indications
Dosage
Adults
For osteoarthritis and chronic musculoskeletal pain: 60 mg once daily. For acute conditions, max 120 mg for limited duration.
Elderly
No dose adjustment generally required, but caution is advised due to increased risk of adverse effects.
Renal_impairment
Mild to moderate renal impairment: No dose adjustment. Severe renal impairment (creatinine clearance <30 mL/min): Not recommended.
How to Take
Tory 60 mg tablet should be swallowed whole with water, with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Etoricoxib is a selective cyclooxygenase-2 (COX-2) inhibitor. It inhibits prostaglandin synthesis by blocking the COX-2 enzyme, which is primarily responsible for the production of prostaglandins that cause pain, inflammation, and fever. Unlike non-selective NSAIDs, it spares COX-1, which is involved in protecting the stomach lining.
Pharmacokinetics
Onset
Within 24 minutes for acute pain, within 24 hours for chronic conditions.
Excretion
Primarily excreted in urine (70%) and feces (20%) as metabolites.
Half life
Approximately 22 hours.
Absorption
Well absorbed orally, with an absolute bioavailability of approximately 100%. Peak plasma concentrations reached in about 1 hour.
Metabolism
Extensively metabolized by cytochrome P450 enzymes (CYP3A4 primarily) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to etoricoxib or any component of the tablet
- Active peptic ulceration or gastrointestinal bleeding
- Severe hepatic dysfunction
- Severe renal dysfunction (creatinine clearance <30 mL/min)
- Congestive heart failure (NYHA Class II-IV)
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- Pregnancy and lactation
Drug Interactions
Lithium
May increase plasma lithium levels, leading to toxicity.
Warfarin
May increase INR and risk of bleeding.
Diuretics
May reduce natriuretic effect and increase risk of renal impairment.
Methotrexate
May increase plasma methotrexate levels and toxicity.
ACE Inhibitors/ARBs
May reduce antihypertensive effect and worsen renal function.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal disturbances, renal toxicity, and cardiovascular effects. Treatment is symptomatic and supportive; there is no specific antidote. Gastric decontamination may be considered within the first hour of ingestion.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. NSAIDs, including etoricoxib, should be avoided in the third trimester due to risk of premature closure of the ductus arteriosus and renal dysfunction in the fetus. Excreted in breast milk, thus not recommended during breastfeeding.
