Description
Overview of the medicine
Tory 90 mg tablet contains Etoricoxib, a selective COX-2 inhibitor used to relieve pain and inflammation associated with various musculoskeletal conditions.
Uses & Indications
Dosage
Adults
Rheumatoid arthritis: 90 mg once daily. Ankylosing Spondylitis: 90 mg once daily. Acute pain (e.g., post-surgical dental pain, other acute pain conditions): 90 mg once daily, for a maximum of 3 days for dental pain or 5 days for general acute pain.
Elderly
No dosage adjustment generally required, but caution is advised due to increased risk of adverse effects.
Renal_impairment
Patients with severe renal impairment (creatinine clearance <30 mL/min) should avoid this medicine. For moderate impairment, caution is advised and dose reduction may be considered.
How to Take
Take orally with or without food. It is recommended to take the lowest effective dose for the shortest possible duration.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2), an enzyme responsible for the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. By inhibiting COX-2, it reduces prostaglandin production, thereby alleviating symptoms.
Pharmacokinetics
Onset
Within 24 minutes for pain relief, longer for chronic inflammation.
Excretion
Mainly excreted in urine (70%) and feces (20%) as metabolites.
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations reached in approximately 1 hour.
Metabolism
Extensively metabolized by the liver, primarily via cytochrome P450 (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to etoricoxib or any component of the tablet
- Active peptic ulceration or gastrointestinal bleeding
- Severe hepatic impairment
- Severe congestive heart failure (NYHA Class II-IV)
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- Pregnancy and lactation
Drug Interactions
Lithium
Increased plasma lithium levels.
Warfarin
Increased prothrombin time/INR.
Diuretics
Reduced diuretic and antihypertensive effects.
Rifampicin
Decreased etoricoxib plasma concentrations.
Methotrexate
Increased methotrexate plasma concentrations, leading to toxicity.
ACE inhibitors/Angiotensin II antagonists
Reduced antihypertensive effect and increased risk of renal impairment.

Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms of overdose are generally mild and may include gastrointestinal disturbances and renal toxicity. Treatment is supportive and symptomatic; hemodialysis is unlikely to be effective.
Pregnancy & Lactation
Contraindicated during pregnancy, especially in the third trimester due to risk of premature closure of the ductus arteriosus. Not recommended during breastfeeding as it is excreted in milk.
