Overview of the medicine
Toujeo SoloStar 300 IU/mL is a long-acting insulin glargine formulation used to improve glycemic control in adults and pediatric patients 6 years and older with diabetes mellitus. It provides a stable, long-lasting glucose-lowering effect over 24 hours with a lower risk of nocturnal hypoglycemia compared to U-100 insulin glargine.
Uses & Indications
Dosage
Adults
Individualized, usually once daily at the same time each day. For Type 1, initial dose is approx. one-third of total daily insulin dose. For Type 2, initial dose is 0.2 units/kg once daily, then adjusted based on glucose levels.
Elderly
No specific dose adjustment needed, but careful monitoring is recommended due to potential for renal impairment and increased risk of hypoglycemia.

How to Take
Administer subcutaneously once daily at the same time each day. Rotate injection sites within the same region (e.g., abdomen, thigh, deltoid) to reduce the risk of lipodystrophy. Do not administer intravenously or intramuscularly. Do not dilute or mix with other insulins.
Mechanism of Action
Insulin glargine is a recombinant human insulin analogue that lowers blood glucose by stimulating peripheral glucose uptake, especially by skeletal muscle and fat, and by inhibiting hepatic glucose production. Insulin also inhibits lipolysis and proteolysis, and enhances protein synthesis.

Oral antidiabetic agents, ACE inhibitors, angiotensin II receptor blockers, disopyramide, fibrates, fluoxetine, MAO inhibitors, pentoxifylline, pramlintide, propoxyphene, salicylates, somatostatin analogs (e.g., octreotide), sulfa antibiotics, alcohol.
Storage
Unopened: Store in a refrigerator (2°C-8°C). Do not freeze. Protect from light. Opened (in-use): Store at room temperature (below 30°C) or in a refrigerator (2°C-8°C) for up to 42 days. Do not freeze. Keep the pen cap on to protect from light.
Overdose
Overdose can lead to severe and prolonged hypoglycemia, which may be life-threatening. Mild hypoglycemia can be treated with oral glucose or carbohydrate. Severe hypoglycemia may require glucagon injection or intravenous glucose. Close monitoring is essential.
Pregnancy & Lactation
Limited data on Toujeo in pregnancy; use only if potential benefit justifies potential risk to the fetus. Insulin does not cross the placenta. Caution when administered to a nursing mother; insulin is excreted in breast milk but is unlikely to cause adverse effects in the infant due to degradation in the gastrointestinal tract.
