Description
Overview of the medicine
Toxpel 200 mg syrup is an antibiotic containing Cefixime, a third-generation cephalosporin. It is used to treat a wide range of bacterial infections in children, including respiratory tract infections, urinary tract infections, and ear infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Not typically used for adults in syrup form. For children, dosage is based on weight.
Elderly
No specific dosage adjustment needed for elderly unless severe renal impairment is present.
Renal_impairment
For creatinine clearance <30 mL/min, dosage should be reduced by 50%.
How to Take
Toxpel 200 mg syrup should be taken orally. It can be administered with or without food. Shake the bottle well before each use. Use the provided measuring device for accurate dosing.
Mechanism of Action
Cefixime, the active ingredient in Toxpel, exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains essential for cell wall strength and integrity, leading to bacterial lysis and death.
Pharmacokinetics
Onset
Within 1-2 hours for significant bactericidal activity.
Excretion
Primarily excreted unchanged via urine (about 50% of absorbed dose) and bile (about 10%).
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food can delay time to peak concentration but does not significantly alter the extent of absorption.
Metabolism
Not extensively metabolized in the liver; approximately 10-15% undergoes hepatic metabolism.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or any other cephalosporin antibiotic.
- Patients with a history of severe hypersensitivity reactions (e.g., anaphylaxis) to any penicillin antibiotic.
Drug Interactions
Warfarin
May increase prothrombin time (PT) with or without bleeding. Monitor PT/INR carefully.
Carbamazepine
Concomitant use may increase carbamazepine plasma concentrations.
Oral Contraceptives
May reduce the effectiveness of oral contraceptives; use alternative birth control methods.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment is primarily symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use in pregnancy only if clearly needed. Cefixime is excreted in breast milk; caution should be exercised when administered to a nursing woman.
