Description
Overview of the medicine
Trado Plus is a combination medicine containing Tramadol and Paracetamol, used to relieve moderate to moderately severe pain.
Uses & Indications
Dosage
Adults
Initial dose of 1-2 tablets every 4-6 hours as needed, not exceeding 8 tablets in 24 hours.
Elderly
Similar to adults, but dosage adjustment may be needed based on renal/hepatic function. Maximum 8 tablets per 24 hours.
Renal_impairment
Dose interval may need to be extended (e.g., 12 hours) in patients with severe renal impairment (CrCl < 30 mL/min). Maximum 2 tablets per 24 hours.
How to Take
Take orally with or without food. Do not crush or chew the tablet.
Mechanism of Action
Tramadol acts centrally as an opioid agonist and inhibits norepinephrine and serotonin reuptake. Paracetamol acts primarily in the central nervous system to inhibit prostaglandin synthesis.
Pharmacokinetics
Onset
Analgesic effect typically begins within 30-60 minutes.
Excretion
Primarily excreted renally as metabolites.
Half life
Tramadol: approx. 5-7 hours; Paracetamol: approx. 2-3 hours.
Absorption
Tramadol and paracetamol are rapidly and well absorbed after oral administration.
Metabolism
Both are extensively metabolized in the liver (Tramadol via CYP2D6 and CYP3A4, Paracetamol via glucuronidation and sulfation).
Side Effects
Contraindications
- Hypersensitivity to tramadol, paracetamol, or any component
- Acute intoxication with alcohol, hypnotics, opioids, centrally acting analgesics, psychotropic drugs
- Severe respiratory depression
- Severe hepatic impairment
- Epilepsy not controlled by treatment
Drug Interactions
Carbamazepine
Significantly reduces tramadol plasma concentrations.
MAO inhibitors
Increased risk of serotonin syndrome and seizures.
Warfarin and coumarin derivatives
Increased INR, leading to bleeding risk.
Serotonergic drugs (e.g., SSRIs, TCAs)
Increased risk of serotonin syndrome.
CNS depressants (e.g., alcohol, benzodiazepines)
Increased risk of respiratory depression, sedation, coma, and death.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include respiratory depression, seizures, coma, hepatic necrosis (paracetamol component). Treatment involves supportive care, naloxone for opioid effects, and N-acetylcysteine for paracetamol toxicity.
Pregnancy & Lactation
Not recommended during pregnancy (risk of neonatal opioid withdrawal syndrome). Use with caution during lactation; small amounts of tramadol and paracetamol pass into breast milk.
