Overview of the medicine
Trapic 500 mg Tablet contains Tranexamic Acid, which is an antifibrinolytic agent used to prevent or reduce excessive bleeding in various conditions such as heavy menstrual bleeding, nosebleeds, and bleeding after surgery or trauma.
Uses & Indications
Dosage
Adults
For menorrhagia: 1-1.5 g (2-3 tablets of 500 mg) 3-4 times daily for 3-5 days. For other indications, dose varies, typically 1 g 2-3 times daily or as prescribed by a physician.
Elderly
No specific dose adjustment is required unless there is renal impairment. Start with lower doses and monitor renal function.
Renal_impairment
Dosage must be reduced according to creatinine clearance. E.g., CrCl 30-59 mL/min: 500 mg twice daily. CrCl 10-29 mL/min: 500 mg once daily. CrCl <10 mL/min: 250 mg once daily.
How to Take
Take the tablet orally with water, with or without food. Follow the duration of treatment as advised by your doctor.
Mechanism of Action
Tranexamic acid inhibits fibrinolysis by competitively blocking plasminogen binding sites on fibrin, preventing the activation of plasminogen to plasmin. Plasmin is an enzyme that breaks down fibrin blood clots. By inhibiting plasmin, tranexamic acid helps to stabilize blood clots and reduce bleeding.

Antifibrinolytic effect within 30 minutes after intravenous administration; longer for oral.
Excretion
Excreted primarily unchanged in the urine via glomerular filtration. 90% of the dose is excreted within 24 hours.
Half life

Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentration achieved within 3 hours after oral administration.
Metabolism
Minimally metabolized; less than 5% is metabolized.

Contraindications
- Hypersensitivity to tranexamic acid or any component of the formulation
- Active intravascular clotting
- Severe renal impairment (risk of accumulation)
- History of convulsions
- Subarachnoid hemorrhage (risk of cerebral edema and infarction)
- Concomitant use of hormonal contraceptives (increased risk of thrombosis)
Drug Interactions
Urokinase
Reduced efficacy.
Combined hormonal contraceptives
Increased risk of thrombosis (blood clots). Avoid concomitant use.
Thrombolytic agents (e.g., Alteplase, Streptokinase)
Antagonistic effect; reduces efficacy of thrombolytics.
Factor IX complex concentrates, Anti-inhibitor coagulant concentrates
Increased risk of thrombosis. Avoid concomitant use.

Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.

Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Tranexamic acid is excreted in breast milk in small amounts; use with caution during lactation.
