Overview of the medicine
Trastunix is a targeted therapy containing Trastuzumab, a monoclonal antibody that targets the HER2 protein. It is used in the treatment of HER2-positive breast cancer and metastatic gastric cancer.
Uses & Indications
Dosage
Adults
Loading dose: 4 mg/kg IV over 90 mins. Maintenance dose: 2 mg/kg IV weekly or 6 mg/kg IV every 3 weeks (for breast cancer). For gastric cancer, 8 mg/kg loading, then 6 mg/kg every 3 weeks.
Elderly
No specific dose adjustment required based on age.

How to Take
Administered as an intravenous infusion by a healthcare professional. Must not be administered as an intravenous push or bolus. The first dose should be infused over 90 minutes; subsequent doses can be infused over 30 minutes if the first infusion was well tolerated.
Mechanism of Action
Trastuzumab binds specifically to the extracellular domain of the human epidermal growth factor receptor 2 (HER2) protein, preventing its dimerization and downstream signaling, which inhibits the proliferation of HER2-overexpressing tumor cells.

Side Effects
Contraindications

Drug Interactions
Taxanes
Often used in combination, monitor for additive myelosuppression.
Anthracyclines
Increased risk of cardiac toxicity. Avoid concomitant use where possible or monitor cardiac function closely.
Storage
Store vials in a refrigerator at 2°C to 8°C. Do not freeze. Protect from light. Reconstituted solution should also be stored at 2°C to 8°C and used within 28 days.
Overdose
No specific antidote. Management involves supportive care and close monitoring of cardiac and pulmonary function.
Pregnancy & Lactation
Pregnancy Category D. Can cause fetal harm. Avoid use during pregnancy. Advise women of reproductive potential to use effective contraception. It is unknown whether Trastuzumab is excreted in human milk; due to the potential for serious adverse reactions in breastfed infants, advise lactating women not to breastfeed during treatment and for 7 months following the last dose.
