Description
Overview of the medicine
Trazuma 440 mg Injection contains Trastuzumab, a monoclonal antibody used to treat HER2-positive breast cancer and metastatic gastric or gastroesophageal junction adenocarcinoma.
Uses & Indications
Dosage
Adults
Initial loading dose of 8 mg/kg IV over 90 minutes, followed by a maintenance dose of 6 mg/kg IV over 30-90 minutes every 3 weeks. Alternatively, 2 mg/kg weekly. Dosing varies by indication and patient weight.
Elderly
No specific dose adjustment required, but monitor for cardiac function.
Renal_impairment
No specific dose adjustment required.
How to Take
Administer intravenously via infusion pump over the recommended time (e.g., 90 minutes for loading, 30-90 minutes for maintenance). Requires reconstitution with sterile water for injection.
Mechanism of Action
Trastuzumab selectively binds to the extracellular domain of the HER2 protein, inhibiting tumor cell proliferation and mediating antibody-dependent cell-mediated cytotoxicity (ADCC) against HER2-overexpressing tumor cells.
Pharmacokinetics
Onset
Rapidly distributed following intravenous administration.
Excretion
Mainly through proteolytic degradation, not significantly via renal or hepatic excretion.
Half life
Approximately 28 days (variable, dose-dependent).
Absorption
Administered intravenously, resulting in complete systemic bioavailability.
Metabolism
Catabolized like endogenous immunoglobulins, not primarily through hepatic or renal pathways.
Side Effects
Contraindications
- •Hypersensitivity to Trastuzumab or any of its excipients.
- •Severe cardiac dysfunction (e.g., severe uncontrolled heart failure).
Drug Interactions
Paclitaxel
May alter Trastuzumab clearance when co-administered, but generally considered clinically manageable.
Anthracyclines
Increased risk of cardiotoxicity when used concurrently or sequentially with anthracyclines. Avoid concomitant use.
Storage
Store reconstituted solution at 2-8°C (36-46°F) for up to 28 days. Store unreconstituted vials at 2-8°C (36-46°F) and protect from light. Do not freeze.
Overdose
No specific antidote for Trastuzumab overdose. Management should involve supportive care and monitoring of vital signs and cardiac function.
Pregnancy & Lactation
Pregnancy Category D: May cause fetal harm. Not recommended during pregnancy. Women of childbearing potential should use effective contraception. Excreted in breast milk; advise against breastfeeding during treatment and for 7 months after the last dose.
