Description
Overview of the medicine
Triamace cream is a topical corticosteroid used to reduce inflammation, itching, and redness associated with various skin conditions.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area 2-3 times daily, or as directed by a physician. Rub in gently.
Elderly
No specific dosage adjustment is generally required. Use adult dosage with caution, especially if extensive skin areas are involved.
Renal_impairment
No specific dosage adjustment is generally required for topical use due to minimal systemic absorption.
How to Take
For external use only. Apply a thin film to the affected skin area and rub in gently. Do not cover with occlusive dressing unless specifically directed by a doctor. Wash hands thoroughly before and after application.
Mechanism of Action
Triamcinolone acetonide works by diffusing across cell membranes and complexing with specific cytoplasmic receptors. These complexes then enter the nucleus, bind to DNA, and regulate gene expression, ultimately suppressing the immune response and reducing the production of inflammatory mediators (e.g., prostaglandins, leukotrienes). It also exhibits vasoconstrictive properties.
Pharmacokinetics
Onset
Clinical effects typically observed within hours to days of initial application.
Excretion
Metabolites are primarily excreted via the kidneys.
Half life
Not well defined for topical application due to minimal systemic absorption. For systemic triamcinolone, plasma half-life is approximately 2-5 hours.
Absorption
Minimal systemic absorption through intact skin. Absorption may be increased with inflamed skin, prolonged use, or occlusive dressings.
Metabolism
Any absorbed triamcinolone is primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to triamcinolone acetonide or any other component of the cream
- Untreated fungal, bacterial, or viral infections of the skin (e.g., herpes simplex, varicella)
- Rosacea
- Perioral dermatitis
- Acne vulgaris
- Pruritus without inflammation
Drug Interactions
Systemic Drugs
Due to minimal systemic absorption, significant drug interactions with systemically administered medications are unlikely with appropriate topical use. However, prolonged or extensive application, especially under occlusion, may lead to some systemic absorption and potential interactions.
Storage
Store below 30°C in a dry place, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is rare but may occur with extensive or prolonged use, leading to systemic corticosteroid effects (e.g., Cushing's syndrome, adrenal suppression) or local adverse reactions (e.g., skin atrophy). Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use in pregnancy and lactation only if the potential benefit justifies the potential risk to the fetus or infant. Consult a physician before use.

Lab Monitoring
- Not typically required for short-term, localized topical use. For extensive or long-term use, especially in children, monitoring for hypothalamic-pituitary-adrenal (HPA) axis suppression may be considered.
Doctor Notes
- Emphasize proper application technique: thin layer, gentle rubbing, avoid excessive use.
- Advise patients against using on the face, groin, or underarms unless specifically instructed, due to higher absorption and risk of local side effects.
- Counsel on the importance of short-term use and monitoring for signs of skin atrophy or infection.
Patient Guidelines
- Use only as directed by your healthcare provider.
- Avoid contact with eyes, nose, mouth, or other mucous membranes.
- Do not use on the face, groin, or underarms unless directed by a doctor.
- Do not cover the treated area with bandages or other occlusive dressings unless advised by your doctor.
- Inform your doctor if your condition does not improve within 2 weeks or if it worsens.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply extra to make up for a missed dose.
Driving Precautions
Topical Triamace cream is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Keep the affected skin area clean and dry.
- Avoid harsh soaps or irritants on the treated area.
- Moisturize regularly to prevent dryness, but avoid applying moisturizer immediately before or after the cream.
