Tricomix Vaginal Suppository is a combination medicine used to treat various vaginal infections, including those caused by bacteria, fungi, and protozoa (like Trichomonas). It combines metronidazole for protozoal and anaerobic bacterial infections, miconazole for fungal infections, and neomycin for bacterial infections.
Uses & Indications
Dosage
Adults
Insert one suppository deep into the vagina once daily, preferably at bedtime, for 7 to 14 consecutive days or as directed by the physician.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is generally needed due to minimal systemic absorption.
How to Take
Wash hands before and after use. Lie on your back with knees bent and insert the suppository deep into the vagina. It is best to administer at bedtime to allow the suppository to dissolve and act overnight.
Mechanism of Action
Metronidazole disrupts DNA synthesis in susceptible organisms. Miconazole inhibits ergosterol synthesis, an essential component of fungal cell membranes, leading to increased permeability and cell death. Neomycin binds to bacterial 30S ribosomal subunits, inhibiting protein synthesis and causing cell death.

Symptomatic relief may begin within a few days of initiating treatment.
Excretion
Systemically absorbed metronidazole and miconazole are mainly excreted renally; neomycin via feces.
Half life
Systemic half-lives are approximately 6-8 hours for metronidazole, 24 hours for miconazole (if absorbed), and variable for neomycin. However, local action is predominant.
Absorption
Minimal systemic absorption of metronidazole, miconazole, and neomycin occurs from the vaginal route. Local concentrations are high.
Metabolism
Systemically absorbed components are primarily metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole, miconazole, neomycin, or any other component of the suppository.
- •Known history of blood dyscrasias.
- •First trimester of pregnancy (due to metronidazole component).
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia) when consumed with metronidazole.
Lithium
Increased serum lithium levels with concomitant metronidazole use.
Ciclosporin
Increased ciclosporin levels with metronidazole.
Phenytoin/Phenobarbital
Reduced metronidazole efficacy.
Other nephrotoxic/ototoxic drugs
Potential for increased toxicity if significant systemic neomycin absorption occurs.
Warfarin and other oral anticoagulants
Increased anticoagulant effect due to metronidazole and miconazole.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Overdose via the vaginal route is unlikely to cause systemic toxicity due to minimal absorption. However, if accidental ingestion or excessive vaginal use occurs, symptomatic and supportive measures should be taken. There is no specific antidote.
Pregnancy & Lactation
Metronidazole is contraindicated in the first trimester of pregnancy. For the second and third trimesters, and during lactation, use only if clearly needed and the potential benefits outweigh the risks. Consult your doctor before use.
