Description
Overview of the medicine
Trigent 200 mg Capsule contains Aceclofenac, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Uses & Indications
Dosage
Adults
Recommended dose is 100 mg twice daily. The 200 mg capsule may be used once daily in certain conditions or as prescribed by a physician.
Elderly
No dosage adjustment is generally required, but caution should be exercised and the lowest effective dose used.
Renal_impairment
Caution is advised. Start with a reduced dose and monitor renal function. Avoid in severe renal impairment.
How to Take
Take the capsule orally, preferably with or after food to minimize gastrointestinal discomfort. Swallow whole with water; do not chew or crush.
Mechanism of Action
Aceclofenac works by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, which are involved in the synthesis of prostaglandins. Prostaglandins are mediators of pain, inflammation, and fever. By reducing prostaglandin production, aceclofenac reduces pain and inflammation.
Pharmacokinetics
Onset
Pain relief typically begins within 30-60 minutes.
Excretion
Mainly excreted in the urine (approx. 70-80%) as metabolites and in feces (approx. 20%)
Half life
Approximately 4 hours.
Absorption
Rapidly and completely absorbed after oral administration; peak plasma concentrations reached within 1.25 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 2C9 (CYP2C9) into 4'-hydroxyaceclofenac.
Side Effects
Contraindications
- Hypersensitivity to aceclofenac or other NSAIDs (e.g., aspirin)
- Patients with active or history of recurrent peptic ulcer/hemorrhage
- Patients with severe heart failure, hepatic failure, or renal failure
- Third trimester of pregnancy
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Warfarin, other anticoagulants
Increased risk of bleeding.
Diuretics, ACE inhibitors, Angiotensin II receptor antagonists
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, and rarely, diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting, convulsions. In cases of significant poisoning, acute renal failure and liver damage are possible. Management is supportive and symptomatic; gastric lavage or activated charcoal may be considered if recent ingestion.
Pregnancy & Lactation
Avoid during the third trimester of pregnancy due to risk of premature closure of the ductus arteriosus and potential fetal renal dysfunction. Use with caution during first and second trimesters only if potential benefit outweighs risk. Avoid during breastfeeding as aceclofenac may pass into breast milk.
