Description
Overview of the medicine
Trimetazidine is a cytoprotective anti-ischemic agent used for the symptomatic treatment of angina pectoris, vertigo, and tinnitus. It works by optimizing myocardial energy metabolism.
Uses & Indications
Dosage
Adults
100 mg once daily (extended-release tablet), preferably with meals.
Elderly
No specific dose adjustment is required unless there is moderate to severe renal impairment. Careful titration is recommended.
Renal_impairment
In patients with moderate renal impairment (creatinine clearance 30-60 mL/min), the dose should be reduced. Consult physician for dose adjustment. Not recommended for severe renal impairment (creatinine clearance < 30 mL/min).
How to Take
For oral use. Tablets should be swallowed whole with a glass of water, preferably with meals. Do not chew or crush the tablet.
Mechanism of Action
Trimetazidine inhibits the 3-ketoacyl-CoA thiolase (3-KAT) enzyme, thereby shifting cardiac energy metabolism from fatty acid oxidation to glucose oxidation. This improves glucose utilization, reduces oxygen consumption, and maintains ATP levels in ischemic cells, thus protecting against ischemia.
Pharmacokinetics
Onset
Clinical effects may be observed within a few days to a week of regular administration.
Excretion
Mainly excreted unchanged in the urine.
Half life
Approximately 6 hours for immediate release, extended to 7-12 hours for modified/extended release formulations.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration reached within 2 hours for immediate release, with extended release showing sustained levels.
Metabolism
Minimally metabolized; primarily excreted unchanged.

- Parkinson's disease, parkinsonian symptoms, tremors, restless leg syndrome, and other related movement disorders
- Severe renal impairment (creatinine clearance < 30 mL/min)
