How to Take
For nasal administration only. Shake the bottle gently before use. Prime the pump before first use or if not used for more than 2 weeks. Insert the nozzle into one nostril, closing the other, and spray while breathing in gently. Repeat for the other nostril. Avoid spraying into the eyes or mouth.
Mechanism of Action
Triamcinolone Acetonide is a potent glucocorticoid with anti-inflammatory properties. It works by binding to glucocorticoid receptors in the nasal mucosa, thereby inhibiting the release of inflammatory mediators (e.g., histamine, prostaglandins, leukotrienes) and reducing the migration of inflammatory cells (e.g., eosinophils, mast cells). This leads to a reduction in swelling, itching, and mucus production in the nasal passages.
Pharmacokinetics
Onset
Onset of symptom relief can occur within 3-4 hours, with maximal effect seen after 3-4 days of continuous use.
Excretion
Excreted mainly via urine and feces as metabolites.
Half life
The plasma half-life of triamcinolone acetonide is approximately 2-3 hours.
Absorption
Systemic absorption of triamcinolone acetonide from nasal administration is low, typically less than 1-2%. Peak plasma concentrations occur 1-2 hours after administration.
Metabolism
Primarily metabolized in the liver by CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to triamcinolone acetonide or any component of the formulation.
- •Untreated localized infection involving the nasal mucosa.
Drug Interactions
Ketoconazole (and other CYP3A4 inhibitors)
Concurrent use may increase systemic exposure to triamcinolone, requiring caution.
Ritonavir (and other strong CYP3A4 inhibitors)
Potential for increased systemic corticosteroid effects; monitor for adverse reactions.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose with Trispray is unlikely due to its low systemic absorption. Chronic overdose may lead to signs and symptoms of hypercorticism, including Cushing's syndrome. Management involves discontinuation of the drug and symptomatic supportive therapy.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known if triamcinolone acetonide is excreted in human milk; use with caution in nursing mothers.
