Overview of the medicine
Tritin 100 mg Tablet contains Tritinamide, a novel agent used for the management of neuropathic pain, including painful diabetic neuropathy and post-herpetic neuralgia. It works by modulating specific calcium channels in the central nervous system.
Uses & Indications
Dosage
Adults
Initial dose is 100 mg once daily, adjusted based on response and tolerability up to 300 mg daily in divided doses. Max 600 mg/day.
Elderly
Start with 50 mg once daily, gradually increase with caution due to potential for increased sensitivity and renal impairment.
Renal_impairment
For CrCl 30-60 mL/min, reduce dose by 50%. For CrCl <30 mL/min, reduce dose by 75% or administer every other day.
How to Take
Tritin 100 mg Tablet should be swallowed whole with water, with or without food. Do not crush, chew, or break the tablet.
Mechanism of Action
Tritinamide selectively binds to the alpha-2-delta subunit of voltage-gated calcium channels in the presynaptic terminals of neurons, particularly in the dorsal horn of the spinal cord. This binding reduces the influx of calcium ions, thereby decreasing the release of excitatory neurotransmitters such as glutamate, substance P, and norepinephrine. This action ultimately attenuates the hyperexcitability of neurons associated with neuropathic pain.

Onset
Pain relief typically begins within 1-2 weeks of consistent dosing, with full effect seen after 4 weeks.
Excretion
Primarily excreted unchanged in the urine; approximately 98% of the administered dose is recovered in urine.
Half life
6-7 hours, not significantly altered in patients with mild to moderate renal impairment.
Absorption
Rapidly absorbed after oral administration, bioavailability is approximately 90% and is not affected by food. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Minimal hepatic metabolism; less than 5% is metabolized. Primarily excreted unchanged.

Drug Interactions
Antacids
Antacids containing aluminum and magnesium may reduce the bioavailability of Tritinamide. Administer Tritinamide at least 2 hours after antacids.
CNS Depressants
Concomitant use with other CNS depressants (e.g., opioids, benzodiazepines, alcohol) may enhance CNS depression and increase risk of dizziness and somnolence.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe dizziness, somnolence, blurred vision, slurred speech, and lethargy. Treatment is primarily supportive; gastric lavage or activated charcoal may be considered in recent ingestions. Hemodialysis is not effective.

Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether Tritinamide is excreted in human milk; caution is advised when administered to a nursing mother.
