Description
Overview of the medicine
Trizin (Cetirizine) is a non-sedating antihistamine used to relieve symptoms of various allergic conditions such as hay fever, perennial allergic rhinitis, chronic urticaria, and pruritus.
Uses & Indications
Dosage
Adults
10 mg once daily, or 5 mg twice daily. Maximum dose is 10 mg/day.
Elderly
A dose of 5 mg once daily is recommended. In cases of normal renal function, the standard adult dose may be used.
Renal_impairment
For patients with moderate renal impairment (creatinine clearance 31-50 ml/min), 5 mg once daily. For severe renal impairment (creatinine clearance 10-30 ml/min), 5 mg every other day.
How to Take
Trizin can be taken orally with or without food. Tablets should be swallowed whole with water. Syrup can be measured with a dosing spoon.
Mechanism of Action
Cetirizine is a potent and selective antagonist of peripheral H1-receptors. It inhibits the early phase of the allergic reaction mediated by histamine and also reduces the migration of inflammatory cells and the release of mediators associated with the late-phase allergic reaction.
Pharmacokinetics
Onset
Within 20-60 minutes.
Excretion
Primarily renal; approximately 60% is excreted unchanged in urine within 24 hours. A small amount is excreted in feces.
Half life
Approximately 8-10 hours in adults, longer in elderly and renally impaired patients.
Absorption
Rapidly and well absorbed after oral administration; peak plasma levels are reached within 1 hour.
Metabolism
About 10% of cetirizine is metabolized via O-dealkylation, resulting in an inactive metabolite. The majority is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivatives.
- Patients with severe renal impairment (creatinine clearance less than 10 ml/min).

CNS Depressants (e.g., alcohol, sedatives, tranquilizers): Concurrent use may increase CNS depressant effects like drowsiness. Caution is advised.

Theophylline: A small decrease in cetirizine clearance has been observed with concurrent use of theophylline (400 mg once daily); however, this is unlikely to be clinically significant.

Symptoms of overdose may include drowsiness, tachycardia, tremor, headache, and urinary retention. Management is primarily supportive and symptomatic. Gastric lavage may be considered soon after ingestion. There is no specific antidote.

Pregnancy Category B. Studies in animals have not shown harm, but adequate studies in pregnant women are lacking. Use only if clearly needed. Cetirizine is excreted in breast milk; caution should be exercised when administered to a nursing mother.
