Overview of the medicine
Tropidil Plus 0.8/5 Eye Drop is a combination ophthalmic solution containing Tropicamide and Phenylephrine HCl. Tropicamide causes rapid, short-acting mydriasis and cycloplegia, while Phenylephrine provides additional mydriasis and vasoconstriction. It is primarily used for diagnostic ophthalmic examinations, such as fundus examination and refraction.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the conjunctival sac, repeated in 5-10 minutes if necessary. Complete mydriasis/cycloplegia typically achieved within 20-40 minutes.
Elderly
Similar to adults, but use with caution due to potential systemic effects.
Renal_impairment
No specific dosage adjustment required for renal impairment, as systemic absorption is minimal.
How to Take
For ophthalmic use only. After instilling the drops, apply gentle pressure to the nasolacrimal duct for 1-2 minutes to minimize systemic absorption.
Mechanism of Action
Tropicamide is an anticholinergic agent that blocks the muscarinic acetylcholine receptors in the sphincter pupillae muscle and ciliary body, leading to pupillary dilation (mydriasis) and paralysis of accommodation (cycloplegia). Phenylephrine is an alpha-1 adrenergic agonist that directly stimulates alpha-adrenergic receptors in the dilator pupillae muscle, causing mydriasis, and in the conjunctival blood vessels, causing vasoconstriction.
Pharmacokinetics
Onset
Mydriasis: 10-15 minutes; Cycloplegia: 20-30 minutes.
Excretion
Systemically absorbed components and their metabolites are primarily excreted via the kidneys.
Half life
Ocular effects of mydriasis typically last 3-6 hours, cycloplegia 3-8 hours. Systemic half-life not well characterized for ophthalmic use.
Absorption
Rapid ocular absorption; some systemic absorption may occur, particularly in children or with prolonged use.
Metabolism
Tropicamide undergoes minor systemic hydrolysis. Phenylephrine is metabolized by monoamine oxidase (MAO) and catechol-O-methyltransferase (COMT).
Side Effects
Contraindications
- •Hypersensitivity to tropicamide, phenylephrine, or any component of the formulation.
- •Primary glaucoma or a tendency towards narrow-angle glaucoma (angle closure glaucoma).
- •Patients with cardiac disease, hypertension, or hyperthyroidism (due to phenylephrine).
- •Infants and small children (due to higher risk of systemic side effects).
Drug Interactions
Antihypertensive agents
Phenylephrine may reduce the hypotensive effects of antihypertensive agents.
Other anticholinergic agents
Additive systemic anticholinergic effects may occur.
Tricyclic Antidepressants (TCAs)
May potentiate the adrenergic effects of phenylephrine, leading to increased systemic effects.
Monoamine Oxidase Inhibitors (MAOIs)
Concomitant use with phenylephrine may lead to hypertensive crisis.

Overdose
Systemic overdose is rare with ophthalmic use but may manifest as tachycardia, hypertension, headache, central nervous system effects (e.g., confusion, hallucinations, seizures), and respiratory depression. Treatment is symptomatic and supportive, including gastric lavage if ingested, and alpha-blockers for severe hypertension.
Pregnancy & Lactation
Use during pregnancy and lactation only if clearly needed and the potential benefits outweigh the potential risks. Systemic absorption is minimal, but caution is advised.
