Description
Overview of the medicine
Trospium Chloride is an antimuscarinic agent used to treat overactive bladder (OAB) with symptoms of urge urinary incontinence, urgency, and urinary frequency.
Uses & Indications
Dosage
Adults
20 mg orally twice daily or 60 mg extended-release once daily.
Elderly
Same as adults, but caution advised due to potential for renal impairment.
Renal_impairment
For moderate to severe renal impairment (CrCl < 30 mL/min), 20 mg once daily or 30 mg extended-release once daily is recommended.
How to Take
Take orally on an empty stomach at least one hour before a meal or two hours after a meal. Swallow the tablet whole with water, do not crush or chew.
Mechanism of Action
Trospium Chloride is an anticholinergic agent that competitively antagonizes muscarinic receptors on the detrusor muscle of the bladder, leading to relaxation of the bladder muscle and inhibition of involuntary contractions. This reduces urinary urgency and frequency.

May increase serum levels of these drugs or trospium due to competitive inhibition of active tubular secretion.

Symptoms of overdose include severe anticholinergic effects (e.g., severe dry mouth, blurred vision, fast heartbeat, flushed skin, difficulty urinating, constipation, confusion, hallucinations). Treatment is supportive, including gastric lavage and physostigmine if severe anticholinergic symptoms are present.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. It is unknown if trospium is excreted in human milk; caution is advised when administering to a nursing mother.
