Description
Overview of the medicine
Trumega 1000 mg capsule contains highly purified Omega-3-acid ethyl esters (EPA and DHA), essential fatty acids vital for cardiovascular health. It is primarily used to reduce very high triglyceride levels and as an adjunct in maintaining general heart health.
Uses & Indications
Dosage
Adults
For severe hypertriglyceridemia: 4 grams daily (e.g., 4 capsules once daily or 2 capsules twice daily with meals). For cardiovascular risk reduction: 2-4 grams daily.
Elderly
No specific dosage adjustment required, but caution should be exercised in elderly patients with pre-existing conditions.
Renal_impairment
No specific dosage adjustment necessary for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take orally with food to enhance absorption and reduce gastrointestinal side effects. Swallow capsules whole; do not break, crush, dissolve, or chew.
Mechanism of Action
Omega-3 fatty acids reduce hepatic triglyceride synthesis and enhance fatty acid oxidation. They also decrease the synthesis of very low-density lipoprotein (VLDL) triglycerides and modestly increase HDL cholesterol levels. Additionally, they have anti-inflammatory and anti-arrhythmic effects.
Pharmacokinetics
Onset
Reduction in triglyceride levels typically observed within 2-4 weeks.
Excretion
Excreted primarily as carbon dioxide and water through metabolic pathways. Minimal renal excretion of unchanged drug.
Half life
Variable; EPA and DHA are incorporated into cellular lipids and have complex pharmacokinetics. Plasma half-life of fatty acids can be several days.
Absorption
Well absorbed from the gastrointestinal tract, primarily in the small intestine, and incorporated into chylomicrons.
Metabolism
Metabolized in the liver via beta-oxidation, similar to dietary fatty acids. Incorporated into plasma lipids and then into cellular lipid pools.
Side Effects
Contraindications
- Known hypersensitivity to Omega-3-acid ethyl esters or any component of the capsule.
- Patients with known allergy to fish or shellfish (some formulations may contain ingredients derived from fish).
Drug Interactions
Fibrates
Concurrent use of fibrates and omega-3 fatty acids may have additive effects on lipid lowering, but also potential for additive adverse effects.
Anticoagulants/Antiplatelet drugs
May increase the risk of bleeding. Monitor bleeding time and INR (International Normalized Ratio) if co-administered with warfarin or other anticoagulants.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include increased bleeding risk, severe gastrointestinal upset. Treatment is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Omega-3 fatty acids are excreted in human milk; use with caution in nursing mothers.
