Description
Overview of the medicine
Truso-DS 200 mg Suspension is an oral antibiotic belonging to the cephalosporin group. It is used to treat a wide range of bacterial infections, including those of the respiratory tract, urinary tract, ear, nose, and throat. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
Generally not for adults in suspension form; adults typically use tablets. If used, 400 mg once daily or 200 mg every 12 hours.
Elderly
No specific dose adjustment for elderly with normal renal function. Dosage may need adjustment based on renal function.
Renal_impairment
Dosage adjustment required. For creatinine clearance 21-60 mL/min, 300 mg daily. For creatinine clearance <20 mL/min, 200 mg daily.
How to Take
Orally. May be taken with or without food. Shake the suspension well before each use. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Cefixime is a third-generation cephalosporin antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. Cell lysis is then mediated by bacterial cell wall autolytic enzymes such as autolysins; it is possible that cefixime interferes with an autolysin inhibitor.
Pharmacokinetics
Onset
Clinical effect within hours of first dose.
Excretion
Excreted primarily in urine (about 50-60% as unchanged drug) and bile.
Half life
About 3-4 hours.
Absorption
Approximately 40-50% absorbed orally; presence of food delays absorption but does not significantly affect overall absorption.
Metabolism
Not extensively metabolized; primarily eliminated as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics
- History of serious hypersensitivity reaction to penicillin (due to potential cross-reactivity)
Drug Interactions
Warfarin
May increase prothrombin time and INR; monitor coagulation parameters.
Probenecid
Increases cefixime plasma levels by decreasing renal excretion.
Oral contraceptives
May decrease efficacy (though clinical significance is debatable).
Storage
Store dry powder below 30°C, protected from light and moisture. Store reconstituted suspension at room temperature (15-30°C) or refrigerated (2-8°C) as per manufacturer's instructions, and discard after 7 or 14 days.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.

- Liver function tests in prolonged therapy
- Complete blood count (CBC) in prolonged therapy
Doctor Notes
- Emphasize full course completion to prevent resistance.
- Advise parents on proper measurement and administration of suspension.
- Monitor for signs of allergic reactions, especially in patients with penicillin allergy history.
Patient Guidelines
- Complete the full course of treatment, even if symptoms improve.
- Do not save suspension for future infections.
- Shake well before each use.
Missed Dose Advice
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
